Incident Investigation, Analysis, and Costs is the title of a chapter in the 12th edition of the National Safety Council s Accident Prevention Manual Administration Programs. [Pg.217]

Accident and near-miss incident reporting and investigation Fire prevention and protection... [Pg.158]

An incident report mainly means a report about a near-miss incident, where a serious accident could be closely avoided. The main components of a report describe circumstances of the incident, investigation of the causes, reasoning of the causes, and preventive measures taken. There are some incident reporting... [Pg.1859]

Incidents include near misses and accidents. Accidents result in loss. Near misses result in no loss. The purpose of any incident investigation is to determine cause(s). As already mentioned, the circumstances differentiating a near miss from a loss-producing incident or accident may be merely a matter of chance therefore, all incidents can be considered candidates for investigation, since correcting the contributing circumstances may help prevent future incidents. [Pg.92]

There is nothing earthshakingly new in the accident/incident prevention arena. Basic accident/incident prevention techniqnes espoused in this book have been used and modified over decades. Eew, if any, dramatically new approaches have been devised using more modem techniques. Thus, accident investigation has always been accident investigation, no matter the words nsed. However, the tools and processes utilized for accident prevention have experienced modification and evolution as accident prevention has become an integral part of the loss control initiatives of companies. [Pg.6]

Once the types of accidents/incidents occurring have been determined, prevention and intervention activities can be undertaken to ensure that no recurrences will transpire. Even if the company is not experiencing large numbers of accidents/incidents, it still needs to implement activities that actively search for, identify, and correct the risk from hazards on jobsites. Reasons to investigate accidents/incidents include... [Pg.47]

Another way to maintain a worksite hazard analysis is to investigate accidents and near-miss incidents so that their causes, and means for their prevention, are identified. By analyzing injury and illness trends over a period of time, patterns with common causes can be identified and prevented. [Pg.70]

Each accident/incident should be methodically analyzed using an accident investigation/root cause analysis approach. Because many root cause analysis methods exist, it will be the investigator s responsibility to select the appropriate analysis approach (e.g., barrier analysis). Use of proper accident/incident investigation methods and tracking will lead to intervention, which will successfully prevent further occurrence of these occupational accidents and incidents. [Pg.473]

Recommended preventive actions should make it very difficult, if not impossible, for the incident to recur. The investigative report should list all the ways to foolproof the condition or activity. Considerations of cost or engineering should not enter in at this stage. The primary purpose of incident investigations is to prevent future occurrences. Beyond this immediate purpose, the information obtained through the investigation should be used to update and revise the inventory of hazards, and/or the program for hazard prevention and control. For example, the Job Safety Analysis should be revised and employees retrained to the extent that it fully refiects the recommendations made by an incident report. Implications from the root causes of the accident need to be analyzed for their impact on all other operations and procedures [6]. [Pg.256]

Incidents and accidents, whether they cause damage to property or more seriously injury and/or ill-health to people, should be properly and thoroughly investigated to allow an organization to take the appropriate action to prevent a recurrence. Good investigation is a key element to making improvements in health and safety performance. [Pg.331]

The purpose of both accident and near miss incident investigation is to carry out an investigation into the undesired event, to determine what happened, and what can be done to prevent a similar event recurring. If positive preventative measures are not taken after an accident or near miss incident, the probability of recurrence is great. The investigation procedure would lead to the root causes of the event and help determine what steps should be taken to prevent a similar accident or near miss incident. [Pg.149]
