Incident Investigation photographs

This useful companion disk contains root cause analysis examples, predefined tree examples, practical checklists that can be customized, and incident evidence photograph examples. It includes a quick checklist for investigators traveling to an incident, examples of methodologies that may be usefiil in training the onsite team, and checklists and samples from the text that can be printed out at the incident site to help organize the team s work. 

In addition to the incident investigation team, there will be others with stakes in the data. Additional samples and copies of photographs will often be required. 

Incident investigation involves varying levels of photographic expertise. For most minor incidents, the team or a company employee can adequately meet the photographic needs. Incidents that are more serious require an experienced individual, such as a forensic specialist, who systematically documents the scene, equipment involved, damage, evidence collection, and position data. For specialized photographic needs, the services of a professional commercial photographer or other specialists are necessary and are justified. Examples of such needs include ... 

Taking multiple exposures is a technique of professional commercial photographers. Most of the exposures actually shot by the professionals are never actually used. It would be unrealistic to expect nonprofessional photographers (such as incident investigation team members) to outperform professionals in this aspect. It is therefore wise to take multiple exposures of essential shots from several angles at various settings. 

Appendix 15.2 provides a checklist of headings, which may assist in the collection of information. It is not expected that all accidents and incidents will be investigated in depth and a dossier with full information prepared. Judgement has to be applied as to which incidents might give rise to a claim and when a full record of information is required. All accident report forms should include the names of all witnesses as a minimum. Where the injury is likely to give rise to lost time, a photograph(s) of the situation should be taken.
