Training programs incident investigation

Amendments to the Clean Air Act require employer-employees consultation to develop and implement PSM program elements and hazard assessments. Section 304 requires employee training and education on the findings of PSM incident investigations. 

When designing an incident investigation and analysis process, a determination must be made concerning the causation model on which the process is to be based. What the designer of the process believes are the facts about incident causation has to be established before an instmctional guide can be written or a training program developed. Consider these extremes. 

In the guidelines to be published and in training programs to be conducted, the purposes and objectives of incident investigation should be established and communicated in a language understood in the organization, particularly in relation to its established safety policies and procedures. Employees must be consulted in the development and implementation of the PSM program. The relevant sections of the underlying legislation also require training employees with respect to processes, procedures, and equipment. They must be kept current and specifically informed of the results of incident investigations ( 12). 

Case 3. Employee participation at a farm machinery manufactnrer of 675 employees includes active membership on several committees and subcommittees. Members change on a voluntary, rotational basis. Committees conduct routine plant-wide inspections and incident investigations. Employees also are involved in conducting training on a variety of safety and health topics. Maintenance employees are revising the preventive maintenance program.
