Competency stmcture for functional safety During the discussions on lEC standards, it was seen that there should be a responsible authority to implement the safety principles. For organizations, a company safety authority (CSA) is charged with this responsibility. Core principles endorsed by senior management are collectively referred to as strategic competency principles, which are based on a multi-tiered approach to demonstrating functional safety capability. The occupational exposure limits assigned to phosgene may vary not only from country to country, but also from authority to authority. They are not only subject to change with the passage of time, but may also vary between different work environments. Some industrial companies follow their own standards, where these are more stringent than those set by the health and safety authorities [40]. Clearly, there are no sharp boundaries at which concentrations of phosgene are "safe" and at which some are "dangerous" to the human...

To define the safety health and environmental (SHE) responsibilities, accountabilities, and authority to act from the CEO to all subordinate line managers and employees, and further, to appoint all levels of management responsible, and accountable, for the implementation of the company safety system, in their areas of responsibility. 

Our initial round of interviews in the U.K. nuclear power industry was conducted in July 1999 with the British nuclear safety authorities— the Health and Safety Executive (HSE) Nuclear Installations Inspectorate (Nil)— and the private nuclear generating company in England (British Energy). A second round of interviews was conducted in January 2000 to follow up on open issues. Additional background information was obtained from discussions with trade union representatives and a local U.K. trade press correspondent. The U.K. interviewees were given considerable latitude to describe the process and impacts of industry restructuring in their own terms, in part because a sensitive audit was still pre-decisional at the time of the interviews. 

During the early stages of investigation, the nominated investigation team strives to visit the accident site as soon as possible. Initial data (obtained from the victim s company, police and occupational safety authorities) is supplemented by discussions with those present at the time of the accident and the company s representatives. If necessary, the investigation team can request an expert opinion on the technical details, from some outside organisation. 

The control measures and the procedures which go with them must be recorded so too must the results of monitoring. In some cases, for example with cancer-causing substances, these records must, by law, be kept for maity years, and handed to the responsible government authority if the company closes. Check with your occupational health and safety authority on the requirements of any specific legislation on carcinogenic substances. 

The report was critical not only of the operating company, but of the regulator who had oversight of this operation. Consequently, one of the report s recommendations was to transfer regulatory authority for installations of this type to the National Offshore Petroleum Safety Authority (NOPSA).

All SR incident reports, defect reports, and records of SR system demands will be copied to the Company Functional Safety Manager, who will maintain a register of failures/incidents. A 6-monthly summary (identifying trends where applicable) will be prepared and circulated to Project Managers and Technical and Safety Authorities.

To deal with uncertainties related to data collection and overcome the less-than-adequate data problem in this research, factual data from accident investigation reports and case studies were interpreted and then organised to fit the scheme presented in Figures 1,2 and 3. These accident reports were collected from insurance companies, regulatory bodies, commissions of inquiry, investigation boards and industry experts, such as MARSH Inc., the Australian Department of Industry and Resources (DoIR), the Australian National Petroleum Safety Authority (NOPSA), the Brazilian Nation ... 

Available information includes inspection and crash data, review and enforcement case data, records of letters sent by FMCSA, and company safety profiles. The FMCSA Portal will also allow companies to designate authorized users to administer online access to their company safety data by approving individual accounts for users. 

If some facets of the safety effort are not going well, this individual will usually be held accountable even though the authority to rectify the existing problem may not exist. Usually the safely person comes from a staff position, which seldom allows him or her to interfere in any way with the line function of production. Without some authority to impact line function when necessary, the safety professional has little clout as to worksite implementation of the companies safety and health effort. Accountability must go beyond the safely professional. Dan Petersen, a noted safety expert, espouses that what is desired is safe production. ...
