Bum wounds

Human growth hormone, used as a human pharmaceutical, is approved for only one indication in the United States, treatment of growth failure owing to hGH deficiency, a condition known as pituitary dwarfism. However, clinical trials are under way to test its efficacy in Turner s syndrome, bums, wound healing, cachexia, osteoporosis, constitutional growth delay, aging, malnutrition, and obesity. 

Dextrans are produced commercially for use as plasma substitutes (plasma expanders) which can be administered by intravenous injection to maintain or restore the blood volume. They can be used in applications to ulcers or bum wounds where they form a hydrophilic layer which absorbs fluid exudates. 

An example of how these materials can be assembled into a useful artificial soft biologic is artificial skin. The use of artificial skin allows for prompt closure of the largest of bum wounds, thereby limiting wound contracture and providing permanent coverage with a readily available, easily stored skin replacement material. Key characteristics of artificial skin are biocompatibility and resistance to infection.

Quantitative biomarkers of dermal bum exposure are generally similar to those seen after acute oral exposure (see Section 2.2 for details). Qualitative biomarkers are also generally similar, but differ on a few relatively obvious points. In white phosphorus bum exposures, gastrointestinal upset is less common and generally less severe, and phosphorescent particles are frequently visible in the area of a white phosphoms bum rather than in vomitus and/or feces. Also if white phosphoms is still present in the bum wound following cleansing procedures, white phosphoms - induced bums may re-ignite spontaneously...

Helichrysum odoratissimum (L.) Less. On the battlefield, the infection risk of an open bum-wound is very high. The zone of residual thermal damage induced by the CO2 laser has the additional effect of sealing the wound surface, minimizing secondary infection. The zero infection rate in this study is encouraging when compared to previous dermabrasion studies of mustard bums in the same animal model in which a small number of infections occurred despite topical antimicrobials (Brown et al., 1998).

Neomycin has been used widely for topical application in a variety of infections of the skin and mucous membranes, including those associated with bums, wounds, ulcers, and infected dermatoses. Such treatment does not eradicate bacteria from the lesions. 

In immunocompromised patients with mucocutaneous HSV infection, intravenous acyclovir (250 mg/vtf every 8 hours for 7 days) shortens healing time, duration of pain, and the period of virus shedding. Oral acyclovir (800 mg five times per day) and valacyclovir (1000 mg twice daily) for 5-10 days are also effective. Recurrences are common after drug cessation and may require long-term suppression. In those with very localized labial or facial HSV infections, topical acyclovir may provide some benefit. Intravenous acyclovir may be beneficial in viscerally disseminating HSV in immunocompromised patients and in patients with HSV-infected bum wounds.
