Accident Type

The following discussion of accident types assists in determining the potential hazards for the job safety analysis. [Pg.209]

Struck-against. A person forcefully strikes an object. [Pg.209]

Contact-by. Contact by a substance or material that is by its very nature harmful and causes injury. [Pg.209]

Contact-with. A person comes in contact with a harmful material. [Pg.209]

Caught-on. A person or part of the person s clothing or equipment is caught on an object that is either moving or stationary. [Pg.209]

You should look at each of these basic accident types to identify procedures, processes, occupations and tasks that present a hazard to cause one of the accident types in the following section. [Pg.76]


Layer of protection analysis (LOPA) is a simplified form of event tree analysis. Instead of analyzing all accident scenarios, LOPA selects a few specific scenarios as representative, or boundary, cases. LOPA uses order-of-magnitLide estimates, rather than specific data, for the frequency of initiating events and for the probability the various layers of protection will fail on demand. In many cases, the simplified results of a LOPA provide sufficient input for deciding whether additional protection is necessary to reduce the likelihood of a given accident type. LOPAs typically require only a small fraction of the effort required for detailed event tree or fault tree analysis. [Pg.37]

While the F-N curve is a cumulative illustration, the risk profile shows the expected frequency of accidents of a particular category or level of consequence. The diagonal line is a line of constant risk defined such that the product of expected frequency and consequence is a constant at each point along the line. " As the consequences of accidents go up, the expected frequency should go down in order for the risk to remain constant. As the example illustrates, if a portion of the histogram sticks its head up above the line (i.e., a particular type of accident contributes more than its fair share of the risk), then that risk is inconsistent with the risk presented by other accident types. Data for the CFOl are compiled from various federal, state, and local administrative sources, including death certificates, workers compensation reports and claims, reports to various regulatory agencies, medical examiner reports, police reports, and news reports. Source documents are matched so that each fatality is counted only once. To ensure that a fatality occurred while at work, all information is verified from two or more independent source documents or from a source document and a follow-up questionnaire. Approximately 30 data elements are collected, coded, and tabulated, including information about the worker, the fatal incident, and the machinery or equipment involved.
