Written Forms

While some of the systems described above involve written forms, safety and health employee program managers and safety professionals or other appropriate individuals should feel free to design their own hazard reporting forms tailored to their worksite and employees. The flexibility in design can allow for voluntary anonymity by making employee self-identiflcation optional. A box for the forms should be installed along with a specific location for the forms.

Supplier Notification. In 1989 and subsequent years, suppliers of facilities in SIC codes 20-39 are required to develop and distribute a notice if the mixtures or trade name products that they manufacture or process, and subsequently distribute, contain listed toxic chemicals. These notices are distributed to other companies in SIC codes 20-39 or to companies that sell the product to facilities in SIC codes 20-39. tf a Material Safety Data Sheet (MSDS) is not required for the mixture or trade name product, the notification must be in written form (i.e., letter or attachment to a MSDS). Otherwise, the notice must be incorporated into or attached to the MSDS for that product. The supplier notification requirement began with the first shipment of a product in 1989 and must accompany the first shipment each year thereafter. In addition, a new or revised notice must be sent if a change occurs in the product which affects the weight percent of a listed chemical or if it is discovered that a previous notice did not property identify the chemicals or the percentage by weight. For more information on supplier notification, see Appendix E. 

To state in written form, the precise meaning, nature, or characteristics of something. 

In their more familiarly written form NO(OH)2+ = H2N03+ and N(OH)3++ = H3N03++ are respectively the nitracidium and hydronitracidium ions. For further discussion of early theories of species existing in MA the reader is referred to Urbanski (Ref 74, pp 12—14). Modern concepts will be examined in detail in Section VIII... 

Chemical symbol A way of expressing an element in written form. It represents one atom and often is the first one or two letters of the name of the element. 

Standards and guidelines should be established in written form for all dmg regulatory functions. These tools should then be used to guide regulation practice, as well as being made publicly available in order to ensure the transparency of the dmg regulatory process.

This information is normally shown as a superscript (atomic mass) and a subscript (atomic number) to the symbol for the element. Hence 1%C represents the isotope of carbon (atomic number 6) with the atomic mass of 14. In practice the subscript is often omitted because the atomic number is unique to the element that is represented by the appropriate letter (e.g. C for carbon). My graduate advisor told me something many times that I now tell my students Writing is thinking. In order to truly communicate a scientific idea in a precise written form, one really needs to think long and hard about the best way to accomplish that goal. 

In general, standardization ol questionnaires and their items helps to simplify quantitative comparisons between individuals and conditions but simultaneously curtails each individual statement and may force it into inappropriate categories. Moreover, allocating the same weight to all the items m a questionnaire, such as all pairs of opposite toms m a semantic differential, may iron out the effects of substances and obscure mterestmg individual effects. A freely formulated description of drug action, be it m oral or written form, should therefore be included m studies of this land... 

Most of these setpoints are fixed by the supplier, detailed in the purchase order, or are part of the installation qualification file. If the setpoints are fixed by the supplier, there are no acceptance criteria unless the supplier documents the setpoint in written form. When available from supplier s documents or when specified in the bill of order or the installation qualification report, the observed setpoints shall be compared with the documented ones. Any detected discrepancy is mentioned for investigation, justification, approval, or correction. When reported parameters are neither specified nor approved, they shall be mentioned as not specified. 

They suggested that an alternative way of increasing GPs effectiveness would be to channel appropriate information on handicap and facilities available in their locality to them in written form, a tactic also used by some CATs, although to date there is no evidence that such strategies are effective.

The name derives from hentriacontane, a hydrocarbon of the paraffin series CH3(CH2)29CH3 present in petroleum, many natural waxes, and the pheromones released by the Hentriacontane philosophers. The present text contains the written form of 31 of the presentations, and is representative of the coverage of the meeting. The collection of papers is also a good indication of the state-of-the-art of this rapidly moving field. 

The purpose of this Appendix is to provide a description of SI units along with rules for conversion and rules for usage in the written form. Conversion factors are given with one table presenting a full detailed list with extensive footnote explanations and another table giving conversion factors in a simplified form for units commonly encountered by chemical engineers. 

Compared with questionnaires, interviews avoided the need for respondents to commit significant time to providing answers in written form. Questionnaires were also not deemed appropriate because specific responses may require clearance from administrative hierarchies within organisations. Within the context of this PhD research project, compared with interviews, questionnaires would create particular uncertainty in terms of interviewee response rate and time.
