Elevated human chorionic gonadotropin (hCG) levels are seen in pregnancy, trophoblastic diseases, and germ cell tumors. It is a useful tumor marker for tumors of the placenta (trophoblastic tumors) and some tumors of the testes. It is also useful for diagnosing and monitoring pregnancy (see Chapter 54 for a discussion of hCG and pregnancy). 

Also, hCG is useful in identifying patients with trophoblastic tumors and, together with AFP, in detecting non-seminomatous testicular tumors. Levels of hCG correlate with the tumor volume and disease prognosis. The presence of hCG in seminoma may indicate the presence of choriocarcinoma. Because hCG does not cross the blood-brain barrier, the normal cerebrospinal fluid-to-serum ratio is 1 60. Higher levels in cerebrospinal fluid may indicate metastases to the brain. Furthermore, the response to therapy for patients with central nervous system metastasis may be indicated by monitoring the hCG level. 

In the past decade several lines of evidence have shown that human chorionic gonadotropin (hCG) is a thyroid stimulator and may cause hyperthyroidism. The basis for the thyrotropic effect of hCG is the structural similarity of hCG to TSH (similar a subunits and unique /3 subunits). In hyperthyroid patients with very high hCG levels, serum TSH may be inappropriately detectable owing to the weak crossreactivity of hCG in the radioimmunoassay for TSH. In patients with hyperthyroidism caused by trophoblastic tumors, serum hCG levels usually exceed 300 units/mL and always exceed 100 units/mL. The mean peak hCG level in normal pregnancy is 50 units/mL. On a molar basis, hCG has only 1/10,000 the activity of pituitary TSH in mouse bioassays. Nevertheless, this thyrotropic activity may be very substantial in patients with trophoblastic tumors, whose serum hCG concentrations may reach 2000 units/mL. 

Human placental lactogen (HPL) is a 22-kD protein with partial homology to growth hormone. HPL is secreted by syncytiotrophoblastic cells in choriocarcinoma and intermediate trophoblasts in a testicular trophoblastic tumor that is a counterpart to a uterine placental site trophoblastic tumor.

This tumor is composed of neoplastic implantation site intermediate trophoblasts that inhltrate myometrium. hPL and HLA-G are typically expressed. Placental site trophoblastic tumors have an index of 14% (+/- 7%) PSTTs can also contain scattered hCG-expressing syn-cytiotrophoblasts, but they are rare and haphazardly situated, not intimately admixed with mononuclear trophoblasts as is typical of choriocarcinoma. Biopsy or curettage material may suggest the presence of PSTT or placental site nodule. Both the immunophenotype and proliferative index of these lesions differ, with preferential expression of hPL and HLA-G in PSTT, as opposed to FLAP and p63 in placental site nodule. 749...

A dose-dependent decrease in trophoblast tumor cell proliferation was observed in estrogen receptor-positive Jeg3 cells treated with flaxseed extract or certain flaxseed extract fractions, with two fractions producing a 58 to 86% decrease in the production of progesterone. Some extract fractions showed a stimulating effect on hormone production and cell proliferation (Waldschlager et al. 2005).
