Etoposide is administered orally and parenterally in lung cancer (small cells), acute non-lymphoblastic leukaemia and also in Hodgkin s disease, whereas Teniposide is given by intravenous infusion in Hodgkin s disease, and certain forms of cerebral tumours and cancer of the urinary bladder. Etoposide and Teniposide have a cytostatic effect in the S-phase and G2-phase of the life cycle of the cell and differ from other podophyllin derivatives by not causing accumulation in the metaphase but prevent the cell from mitosis or destroy the cells that undergo mitosis.

Vinflumine (Javlor ) is a second-generation Vinca alkaloid. It is more active than the nonfluorinated parent compound (vinorelbine) in several cancers (Figure 8.7). Vinflumine is currently in Phase III clinical trials as a chemotherapeutic agent against a variety of cancers (metastasic breast cancer, small cell lung cancer, and bladder cancer). This drug inhibits mitotic assembly, via inhibition of tubulin polymerization in microtubules, a major element of the cytoskeleton. Effects of fluorine substimtion on tubulin affinity or on metabolism are not responsible for the increased efficiency and decreased toxicity. Docetaxel is a semisynthetic taxane derived from the European yew tree. Its mechanism of action, metabolism, and elimination are identical to those of paclitaxel. It is approved for use as second-line therapy in advanced breast cancer and non-small cell lung cancer, and it also has major activity in head and neck cancer, small cell lung cancer, gastric cancer, advanced platinum-refractory ovarian cancer, and bladder cancer. Its major toxicities are listed in Table 54-4. 

Recently Moody et al.,22) discovered that the C-terminal partial sequence of bombesin and bombesin-like peptides (BLPs) can function as autocrine growth factors in human small-cell lung cancer (SCLC) cell lines. 

In this context, it should be noted that one of the most aggressive types of cancer, small cell lung carcinoma, represents, in fact, a result of malignant transformation of the lung neuroepithelial cells. These tumor cells still produce the same neuromediators [21, 22] and contain both NADPH oxidase and the H202-stimulated channels [8]. Even more, malignant transformation was found to be oxygen-dependent [23].

The two taxanes currently in use are paclitaxel, which is used in the treatment of ovarian cancer and metastatic breast cancer and docetaxel, which is used to treat metastatic breast cancer, small cell lung cancer and prostate cancer.
