Chapter 24 focuses on internal and external reviews of the process safety program implementation to ensure that the level of risk to the workers and the public and environment does not increase. The reviews also verify and validate that accepted safety risk is continually minimized and that government requirements are met or exceeded. The chapter references a key inspection handbook that contains valuable information for both operations and safety personnel. 

Today s safety professional has moved beyond the standard measurement of safety performance in terms of the number of accidents or injuries and recordable injury and illness rates. More sophisticated techniques that involve safety performance measurement systems, ongoing tracking of results, and continual improvement processes have replaced the early measurements. While today s safety metrics still include accident and illness performance, they also encompass behavior-based safety, safety program implementation performance, and insurance costs and losses. 

This list is not intended to be comprehensive but provides ideas on documentation that should be reviewed within the scope of a school safety audit. Reviewing such documents will provide an initial indication of how much safety program implementation activity has occurred. For example, if it is found that there are no documented safety inspections, then the inspection aspect of the school safety program is lacking in implementation. 

Employee participation. As important as management commitment and its impact on effective safety program implementation and performance is employee involvement. 

Although the traditional point of reference for safety interlock systems is a hard-wired implementation, a programmed implementation is an alternative. The potential for latent defects in software implementation is a definite concern. Another concern is that solid-state components are not guaranteed to fail to the safe state. The former is addressed by extensive testing the latter is addressed by manufacturer-supplied and/or user-supplied diagnostics that are routinely executed by the processor within the safety interlock system. Although issues must be addressed in programmable implementations, the hard-wired implementations are not perfect either.

There are at least a couple of conclusions we should draw from this example. If you are going to use safety alerts, even though your intentions are honorable, you may offend the injured party, the folks who took part in the investigation, and others. Be prepared for the fall-out. Also, keep in mind that you need to talk the talk and walk the walk. If an incident occurs and a corrective action is indicated, it behooves you to implement some level of corrective action. If you do not implement a sound corrective action, your company likely has an ineffective safety program to go along with a variety of outstanding lawsuits. 

A properly designed and implemented site-speeifie HASP provides the basis for proteetion of workers, visitors, and the publie. The HASP is a requirement at all HAZWOPER sites [1] and will likely soon beeome a requirement at all sites. However, before we begin development of the HASP there is a lot of work to do. The following diseussion will outline the diflferenees between a health and safety program and a HASP doeument. 

Not all existing procedures or program elements of the overall health and safety program need to be incorporated into the HASP. For example, if noise is a hazard, the plan does not have to cite the entire hearing conservation program. Procedures already established elsewhere may be referenced, as applicable. In another example, if a confined-space-entry procedure is required, the HASP could reference the particular procedure which is part of the overall program. The next step would be to identify confined spaces at the worksite where the procedure applies, and then provide appropriate implementation procedures (e.g., conditions to be monitored, evaluation of the space, issuance of an entry permit). If special operational procedures apply to the worksite, they can be attached to the HASP using an appendix. 

Ensure that the contractor/subcontractor can implement a comprehensive health and safety program in compliance with applicable regulations, including accident prevention programs, medical surveillance, training, work practice controls, use of personal protective equipment, and so on.

The most effective means of implementing a safety program is to make it everyone s responsibility in a chemical process plant. The older concept of identifying a few employees to be responsible for safety is inadequate by today s standards. All employees have the responsibility to be knowledgeable about safety and to practice safety.

For this reason, it makes sense to examine the costs and benefits of making improvements in your environmental, health, and safety programs. Make the HSE area part of your company s planning process, so that change can be proactive rather than reactive. Develop a schedule of implementation, as it is difficult to tackle all of these areas at once. You will need to decide whether to develop the expertise internally or seek outside assistance to devise and accomplish your plan. Training is readily available if you want to do it yourself, and there are many capable consulting firms ready to assist. 

A process safety program must be relevant to the company and its operations. The program must also be relevant to employees personal safety and success. The link between safety actions and safety outcomes is most obvious in operations that directly handle highly hazardous materials. Unfortunately, that nexus may not be as obvious in the systems that support and are otherwise indirectly associated with hazardous materials. Appropriately developed, implemented, and communicated process safety metrics can help educate personnel on the importance of different aspects of the process safety system.
