The commonly used management systems directed toward eliminating the existence of hazards include safety reviews, safety audits, hazard identification techniques, checklists, and proper application of technical knowledge. 

Frank Lees [17] has cataloged and presented a number of checklists in Hazard Identification and Safety Audit. Lees states One of the most useful tools of hazard identification is the checklist. Like a standard or code of practice, a checklist is a means of passing on hard-won experience. It is impossible to envisage high standards in hazard control unless this experience is effectively utilized. The checklist is one of the main tools available to assist in this.

Identification can be as simple as asking what-iP questions at design reviews. It can also involve the use of a checklist outlining the normal process hazards associated with a specific piece of equipment. The major weakness of the latter approach is that items not on the checklist can easily be overlooked. The more formalized hazard-assessment techniques include, but are not limited to, hazard and operability study (HAZOP), fault-tree analysis (FTA), failure mode-and-effect analysis (FMEA), safety indexes, and safety audits.

The first task, hazard identification, is crucial in process system safety analysis, because the effectiveness of the other two tasks depends on it. The traditional methods for identifying hazards during the 1960 s (including process reviews , codes of practice , checklists , and safety audit ) were no longer considered adequate in the 1970 s. There was a need for a technique which could anticipate hazardous problems, particularly in areas of novelty and new technology where past experience was limited.

Workplace safety analysis — Similar to a job safety analysis, a workplace safety analysis involves a careful review of each area of the workplace, including the office, dock, yard, and shop. It should involve a review of lighting, floors, aisle-ways, noise levels, storage areas, and overall compliance with OSHA standards. Again, an audit checklist is an important item to have before starting the analysis.

Clients requiring Road Safety Audits are responsible for producing a Road Safety Audit brief. Requests for Road Safety Audits sometimes come from clients who are unfamiliar with the process. TMS Consultancy routinely sends out a checklist (shown below) indicating what is required to complete a Road Safety Audit. 

Checklists are provided in HD 19/03 and in the IHT Safety Audit Guidelines. The common problems highlighted in Chapter 5 could be used as the basis for producing checklists. 

In case any of these scenarios develop. Road Safety Auditors are advised to maintain good records of their Road Safety Audit process, including any checklists used during the process, and to spell out precisely what information has been used for Road Safety Audit purposes. In the case of receiving an Exception Report on a problem at an early stage of the Road Safety Audit, Road Safety Auditors are advised to repeat the road safety problem at subsequent stages. 

Risks may be identified through employee identification of them, safely inspections, analysis or accident statistics, and near miss incident audits, checklists and reports. This is not an exhaustive list, however. Once risks are identified, review the process involved from Step 1, within the selected department (Step 2), then evaluate and apply risk control principles in consultation with the department supervisor, employees and health and safety representatives (where they exist). 

Team size and frequency will depend on the size of the property. At the beginning of every shift, employees should inspect their work areas and equipment before starting work to ensure that all defects are noted and corrected. Figure 11.2 is a sample daily audit form. The safety reminder checklist is not a complete list of safety topics and should be structured for your work area or fadhty. Note that the items listed in Figure 11.2arenot only positive work practices, but also government requirements.
