To be successful, a team selected from different groups within the project team should participate in the preparation and review of these plans. In addition, a schedule of the review and approval process for these plans needs to be established, accepted by all reviewers, and distributed before release of the first draft. Reviewers should meet an established schedule for review and submission of comments. A distinction between review and approval authority should be determined. The review process determines if all of the required elements are identified. This review can take considerable time, depending on the complexity of the project. The approval process is important when all comments are incorporated. The key responsible persons associated with the project should accept and approve the changes to the document. 

The inspections and tests are documented to include the date, the name of the responsible person, identification of the equipment, a description of the inspection or test and the rcsult.s... 

Protective equipment may have to be defeated from time to time, but this should only be done after authorization in writing by a responsible person. And the fact that the equipment is out of action should be clearly signaled—for example, by a light on the panel. 

When a boiler may not be shut down for maintenance of the level control chambers isolating valves can be fitted between the water-level control and the steam space. The validation is therefore not complete until there is a detailed description of the method and records of the validation study. A responsible person needs to sign that the method meets the requirements, i.e. it is fit for purpose . The documentation facilitates the consistent application of the method, within its scope and defined performance parameters. This, in turn, helps ensure that when the method is applied in different laboratories or at different times, the measurand is the same and that the measurement results are comparable. Documentation is also required for quality assurance, regulatory and contractual purposes. 

Examples of Quality Audit report forms are shown in Tables 9.5 and 9.6 at the end of this chapter. Table 9.5 shows Report Form 1 which includes a record of what has been examined, so that subsequent audits can examine other aspects of the laboratory s operations. Note that when improvement actions are required, the form records not only what needs to be done, but also by when it must be completed, as shown in Table 9.6 (Report Form 2). The form is signed by the auditor and the responsible person from the area being audited. In the UK, the accreditation body UKAS refers to the corrective action as improvement action . This further emphasizes the continual improvement aspect of the ISO/IEC 17025 and ISO 9001 Standards. The examples shown in Tables 9.5 and 9.6 will probably have to be modified to meet a laboratory s particular requirements. It should also be remembered that both report forms should indicate (usually as a footer or header) the title of the document, its issue date, issue number, who authorized the document, page number and total number of pages.

The results of the analysis made by the sanitary and epidemiological service of the city of Krivoi Rog, revealed that the main reason that provoked the cases of ravishment was the formal attitude of the responsible persons towards their duties, for which they were punished in accordance with the sanitary legislation. 

A mismatch between operator procedures and the automatic control system of the reactor (see also Table 17) was the first active failure identified in this scenario. This precursor was still present mainly due to a shortage of people. Literally it was said that the pressure relief valve would open if the wrong value was inserted into the reactor s control system. The second precursor was the failure of the pressure relief valve (see also Table 17), which was not known to the responsible person who decided to ignore the difference between procedures and control system. The pressure relief valve failed, because resins stuck in the valve after it was used for the first time. Consequently the second time the valve was opened it was at a much higher pressure due to the build up of resins in the valve. If this second precursor had not been observed in time by damp on the pipes situated above the pressure relief valve or by the alarms in the control room a possible accident scenario existed. This was especially dangerous as the alarms in the control room are often ignored because of the high incidence of false alarms (see also Table 17), which was the third precursor present. 

The absurdity of the prohibitionist contention is shown by the experience of London and other European cities. Only authorized personnel should be allowed to handle the material and this handling should be done under the supervision of a responsible person. Access to stored material should be controlled. The conception of freedom of the will I allude to raises many problems.2 Obvious questions concern the understanding of the phrase "to have the will one wants to have." 1 believe there are two related but distinct questions here. One question concerns what it is to possess freedom of the will, to be a free and responsible person.
