The rate of cholesterol esterification in plasma is not correlated with HDL concentration (A12, R17, S45, S58, Wl, W2) but is correlated with the concentration of VLDL or triglyceride (A12, P8, R17, S58, T7, Wl, W2). Although HDL is the major substrate for LCAT, VLDL and indirectly LDL are the major recipients of the esterfied cholesterol, transferred (it is thought) by lipid transfer protein. Accumulation of esterified cholesterol in the recipient lipoproteins is associated with a decrease in LCAT activity (C7, Fll, F13) that can be relieved by the addition of recipient lipoproteins but not by addition of LCAT substrate (Fll). Hopkins and Barter (H32, H33) have explained these observations by showing that the depletion of HDL esterified cholesterol by transfer to VLDL enhances the capacity of HDL to act as a substrate for LCAT. [Pg.262]

Two distinct receptor groups have been identified for acetylcholine, the nicotinic and the muscarinic groups (Table 11.1). Furthermore, there are at least four subtypes of nicotinic and five subtypes of muscarinic receptors. Nicotinic receptors are ubiquitous and exist at the neuromuscular junctions of skeletal muscles and on ganglion cells in the autonomic nervous system. Nicotinic receptors located on cation-specific ion channels, when opened, evoke fast, transient depolarizations of the recipient cell. Muscarinic receptors are found in smooth muscle receiving parasympathetic innervation and elsewhere, and can be blocked by atropine. Muscarinic receptors are coupled indirectly to slow and fast ion channels via G proteins. [Pg.194]

In a randomized, double-blind study, trivalent, live, attenuated, cold-adapted intranasal influenza vaccine (FluMist) has been compared with intranasal placebo plus a trivalent injected inactivated influenza vaccine (5). The 200 patients were aged 65 years and over and had chronic cardiovascular or pulmonary conditions or diabetes mellitus. During the 7 days after immunization, sore throat was reported on at least one day by significantly more of the FluMist recipients (15 versus 2%). The increased frequency of sore throat may have been attributable to direct or indirect effects of vaccine virus replication. No other symptom was associated with FluMist. These findings were consistent with evaluations of other live, attenuated, cold-adapted influenza vaccine formulations in older adults. However, further studies of the safety of FluMist are warranted. [Pg.1754]

Sometimes it can help to create a voucher with multiple tear-off or cut-off sections, in which one part of the voucher remains with the bearer, while other parts are returned to the voucher agency or sent to a laboratory with the specimens (box 5-6). It can also be useful to leave a space in the voucher for the patient s signature, to ensure that patients actually receive the health service they were supposed to have received.
