Double blind study

A clinical trial to evaluate misoprostol as a protector of normal tissue during a course of XRT in cancer patients suggests a reduction in acute normal tissue injury (215). A randomized, prospective, double-blind study indicates that topical misoprostol, administered as an oral rinse 15-20 min before irradiation using conventional 2-Gy (200 rad) fractions, five days a week over 6—7 weeks, significantly protects the oral mucosa from radiomucositis, a frequently observed normal tissue complication during XRT for head and neck cancer (215). 

The activity of ivermectin against the filarial parasite Dirofilaria immitis in dogs suggested a possible role for the control of filarial parasites of humans (20). It has been extensively tested in human onchocerciasis and is now considered to be the dmg of choice. In a single yearly oral dose, it suppresses microfilariae in the skin and eyes and, in most cases, prevents the progression of the disease to blindness. Table 4 shows the results of a 30-patient double-blind study recorded over one year.

O Malley et al. (1992) conducted a double-blind study combining naltrexone and CBT for alcohohsm. Patients were randomly assigned to participate in cognitive-behavioral coping skills treatment or supportive therapy and to receive 50 mg/day of naltrexone or placebo. Naltrexone-treated patients who received supportive therapy had more continuous abstinence than the other treatment groups. However, naltrexone-treated patients who received CBT had a lower level of craving and lower risk of relapse than the other three groups. This interaction would not have been observed in a study that manipulated only psychosocial treatment or only medication. 

Several CM studies have explored interactions between medication and psychosocial treatments for substance use disorders. The major clinical study underpinning product approval was a randomized, double blind study in which 71 CTCL patients were administered the product at one of two dosage levels (9 or 18 [tg kg 1 day ) overall, 30 per cent of patients experienced an objective tumour response. Serious side effects potentially associated with product administration include acute hypersensitivity-type reactions, vascular leak syndrome and visual impairment. In a double-blind, placebo-controlled phase I study, ospemifene exerted a very weak estrogenic effect on endometrial histology, and no clinically significant changes were seen in endometrial thickness at any dose level (Voipio et al. 2002). In another double-blind study, ospemifene at daily doses of 30 to 90 mg did not stimulate growth of endometrial thickness (Rutanen et al. 2003).

Pharmaceutical Comparison. At least 8 studies to date have examined the effectiveness of hypericum compared to the pharmaceutical antidepressants imipramine, amitriptyline, and maprotiline. Preliminary results indicate that hypericum is equivalent to standard antidepressants in effectiveness (Linde et al. 1996 Vorbach 1997). Similar to the pharmaceutical antidepressants, there is a 10-14 day lag for therapeutic effects of hypericum (Harrer et al. 1994). Indeed, the differences seen between hypericum and placebo groups becomes apparent between 2 and 4 weeks (Sommer and Harrer 1994). Hypericum has been reported to have a more favorable side-effect profile than several pharmaceutical antidepressants as well (Vorbach et al. 1994 Harrer et al. 1994). In double-blind studies, subjects have reported fewer and less-severe side effects.
