Other Forms of Treatment

Traction is typically used to align a bone by a gende, constant pulling action. The pulling force may be transmitted to the bone through skin tapes or a metal pin through a bone. Traction may be used as a preliminary treatment, before other forms of treatment or after cast immobilization. 

If the water is found fit for consumption, with respect to both its mineral and biological content, the problem of sanitization can still arise. Public supply invariably has a very small residual chlorine level. This suppresses biological growth and maintains water quality even when the line is stagnant. As with other forms of treatment, the scale of private supply is usually too small to allow good control of chlorinating equipment.

A 37-year-old woman with refractory idiopathic thrombocytopenic purpura, who had responded dramatically to danazol 600 mg/day after the failure of other forms of treatment, developed a multinodular well-differentiated trabeculovesicular hepatocellular tumor after 5 years. There were no metastases and no invasion of the lymph nodes, and after chemoembolization she received an orthoptic liver transplant. A year later she remained in good condition. 

The treatment of amiodarone-induced hyperthyroidism is difficult. It often does not respond to conventional therapy with carbimazole, methimazole, or radio-iodine. However, corticosteroids and the combination of methimazole with potassium perchlorate have been reported to be effective (52), even if amiodarone is continued (53). Other regimens that have been used include combinations of corticosteroids with carbimazole (54), corticosteroids and benzylthiouracil (55), or propylthiouracil (SEDA-15, 170). Potassium perchlorate has also been used (SEDA-21, 199). Other forms of treatment that have been successful have been plasma exchange and in very severe cases subtotal thyroidectomy (56) or total thyroidectomy (SEDA-15,170 SEDA-17, 220 57). 

Mild Oxidation (Bleached Starch) The starches resulting from mild oxidation are not altered chemically in all cases, extraneous color bodies are oxidized, solubilized, and removed by washing and filtration. These treatments may be used in combination with the other forms of treatment listed in this section. 

In addition to its use in preventing tuberculosis, BCG has been used as an immunostimulant or immunomodulator. The degree of safety of this procedure differs with the technique and the purpose for which it is used. In most areas, the use of BCG to counter cancer has proved disappointing, although it is still used to some extent, generally as an adjunct to other forms of treatment (1-7). Although medications are the primary treatment modality for bipolar disorder, it is important to consider the impact of medications in conjunction with other forms of treatment. For instance, milieu therapy, when inpatient care is necessary, will be most effective after initial medication response has reestablished some degree of cooperation and insight. Psychotherapy, especially cognitive, behavioral, and psycho-educational approaches, is effective with the medication-stabilized patient. Group therapy, which can include families, will often revolve around acceptance of the disease as well as of the need for long-term medication treatment, the side effects, and the implications of noncompliance. In instances of medication resistance or contraindications, ECT should be considered. 

The goal of medication treatment is to reduce symptoms so that the person can function better and benefit more from other forms of treatment, such as individual, group, or family therapy and social or vocational rehabilitation. An important part of such therapy involves educating the patient to prodromal symptoms and effects and side effects of medications. It is also important to address patients beliefs that taking medications means they are sick (and conversely, that not taking medications means they aren t). Especially with paranoid patients, the belief that medications are a means of being controlled by others must be worked through. 

The decreased secretion of TSH from the pituitary is a pan of a generalized hypopituitarism (hat leads to hypothy-loitlism. This type of hypothyroidism can be distinguished from primary hypothyroidism by the administration of TSH In doses. sufficient to increase (he uptake of radioiodinc or to elevate the blood or plasma protein-bound iodine (PBI) as a consequence of enhanced secretion of hormonal iodine (thyroxine). Interestingly. ma.ssive doses of vitamin A inhibit the secretion of TSH. Thyrotropin is used as a diagnostic agent to differentiate between primary and secondary hypothyroidism. Its use in hypothyroidism caused by pituitary deficiency has limited application other forms of treatment are preferable. 

Oral isotretinoin is available for severe acne refractive to other forms of treatment. It is effective but is teratogenic and can have severe side-effects. 

There are very few treatments in medicine where the results can be so readily and easily determined as with peroxide. This gives peroxide therapy a tremendous advantage over any other form of treatment. Either it s working or it isn t. There is usually no in-between. 

Effective, efficient, and comprehensive helping relationships require that social workers recognize other forms of treatments that could potentially be more effective for their clients. Alternative forms of intervention can also serve as a preventive measure to ensure a client s well being. As providers of stress management strategies, relaxation techniques, and psychosocial interventions that can be incorporated into alternative therapies, social workers need to be aware of these techniques and remedies. Keeping abreast of all forms of treatment is important to provide the best possible care for our clients, while discouraging the use of medications and herbal preparations as the only form of intervention when other alternatives can be just as effective.

Echothiophate iodide has found therapeutic appiication for the treatment of giaucoma and strabismus. Echothiophate is applied topically as a solution and is the only irreversible AChEI for the treatment of glaucoma. The decrease in intraocular pressure observed can last up to 4 weeks. Phosphoester AChEls exhibit cataractogenic properties thus, their use should be reserved for patients who are refractory to other forms of treatment (i.e., short-acting miotics, (3-blockers, epinephrine, and possibly, carbonic anhydrase inhibitors). Because of its toxicity, echothiophate is not used for its systemic action. Selectivity of echothiophate for the AChE catalytic site was enhanced by incorporation in the molecule of a quaternary ammonium salt functional group two carbons removed from the phosphoryl group.
