Heparin and its salts are incompatible with many drugs including alteplase, amikacin sulfate, amiodarone hydrochloride, ampicillin sodium, aprotinin, benzylpenicillin potassium or sodium, cephalothin sodium, ciprofloxacin lactate, cytarabine, dacarbazine, daunorubicin hydrochloride, diazepam, dobutamine hydrochloride, doxorubicin hydrochloride, droperidol, erythromycin lactobionate, gentamicin sulfate, haloperidol lactate, hyaluronidase, hydrocortisone sodium succinate, kanamycin sulfate, methicillin sodium, netilmicin sulfate, some opioid analgesics, oxytetracycline hydrochloride, some phenothiazines, polymyxin B sulfate, streptomycin sulfate, tetracycline hydrochloride, tobramycin sulfate, vancomycin hydrochloride, vinblastine sulfate, cisatracurium besylate, labetalol hydrochloride, nicardipine hydrochloride, cefmetazole, sodium ions, and fat emulsion.110 112 

Hydralazine solutions reacts with metals, hence injections must be prepared using nonmetal filters and doses must be withdrawn prior to administration.113114 Percussion or excessive heat may cause an explosion of undiluted isosorbide dinitrate and must be handled carefully. 

Isosorbide dinitrate adsorbs to PVC plastic intravenous infusion sets from solution. Adsorption and loss of potency could be prevented if stored in glass bottles or polyethylene-, nylon-, or polypropylene-laminated bags.115116 

Patients who are receiving ketanserin must be cautious when driving and operating machinery, as it causes drowsiness. 

Labetalol hydrochloride is incompatible with 5% sodium bicarbonate injection. White precipitate is formed immediately in 5% glucose solutions of ffusemide (10 mg/mL), insulin (100 ml.), and thiopentone.117118

The photostability of two polymorphs of nicardipine hydrochloride have been studied using a number of techniques [95]. After irradiation, the drug substance decomposes to a pyridine derivative, and the photodegradation of the /1-form exceeded that of the x-form. It was also found that the color of the two different forms differed with the polymorphic state, but that grinding the two forms lessened the difference in their photochemistries. A correlation between the heat of fusion (measured by differential scanning calorimetry) and the photodegradation rate constant was observed.

Nicardipine, used in the treatment of hypertension, has proved to be an excellent candidate for transdermal delivery as its bioavailability is only 20-33% due the first pass effect in the liver and the short half life requires frequent dosing resulting in poor patient concordance.
