Intolerance definition

An evaluation of the rifaximin tolerability profile observed in almost 1,000 patients from 30 clinical trials was unable to identify a definite pattern of intolerance [33]. Very few adverse events have been reported during short-tem treatment with the drug, the most frequently reported being gastrointestinal in nature (e.g. flatulence, nausea, abdominal pain and vomiting). It is worthwhile to emphasize that the detection of GI adverse reactions could have been difficult in rifaximin trials since the symptoms of the underlying diseases were often similar to the GI complaints observed after drug treatment. 

Severe, active, classical or definite adult RA (ACR criteria) in selected adults who have had an insufficient therapeutic response to, or are intolerant of, an adequate trial of first-line therapy including full-dose NSAIDs. 

In addition to the somatic side-effects of neuroleptics, there are a number of important psychiatric side-effects, such as demotivation or indifference (a direct effect of most drugs, actually part of the definition of the neuroleptic effect). This may mimic the negative features of the illness and may lead to prescriptions of an antidepressant when a reduction in dose or change of antipsychotic may be more appropriate. A second key problem is anxious activation or akathisia. This dose-dependent dysphoric state may lead to an apparent worsening in the clinical picture and accordingly an increase in antipsychotic dose rather than decrease and may be so intolerable as to lead on to suicide. 

This close definition of the client and service remit was true of virtually all of the practitioners interviewed. This segmentation was not an apparently intentional outcome, but a product of a prevalent model of service delivery that in being highly individualized tends to miss the influential interplay of family relationships and roles and their effect on all family members behaviours. It is just possible that a more family-centred approach that takes into account these dynamics, and works with them as they play out between family members, could achieve some alleviation of the near intolerable stresses that families seem to experience. This kind of family systems therapeutic work is not common. It is difficult, resource-intensive work and undoubtedly requires very skilful management given the emotional tenor of such intervention. As many of these practitioners described, intervention with families, even in the limited terms of enlisting help for their client, invited great caution. 

The definitive landmark trial that tested IM in patients with CML was the International Randomized trial of IFN-a and STI-571 (IRIS). In this study, 1106 patients, 553 in each arm, were randomized to receive IM at 400 mg daily or IFN-a with low-dose cytarabine. Non-insulin-dependent diabetes mellitus (NIDDM), also known as late-onset or type II diabetes, affects over 12 million Americans, only about half of whom are aware of their disease (H6). Importantly, most diabetics have the disease for 4-7 years before it is diagnosed. About 50% of men and women aged 65-74 years demonstrate glucose intolerance (i.e., increased glucose levels but below that required for a definitive diagnosis) about 20% of these have NIDDM (C3, B16). NIDDM is a major cause of cardiovascular disease, stroke, renal failure, and blindness it is also associated with accelerated aging. 

Hereditary fructose intolerance affects one in 21 000 persons, and there have been more than 12 severe complications caused by these solutions, several lethal. Since a prior history of fructose intolerance is often not obtained, it has been argued that the use of fructose- and sorbitol-containing infusion fluids must be regarded as offering doubtful advantage but carrying definite lethal risk, and that their use should be discontinued. A modified intravenous fructose tolerance test should at least be carried out before infusions of fructose or sorbitol are giveu (8). 

The first definition of MCS was created by Theron Randolph in 1962. Randolph described MCS as a condition that (1) is acquired (2) includes physical and mental symptoms that can be triggered by chemical exposure (3) has a specific adaptation syndrome (i.e., adaptation to chemicals is followed by chronic illness, withdrawal symptoms upon removal, and shock upon re-exposure) (4) is characterized by a spreading phenomena (i.e., an intolerance to an increasing number of environmental chemicals) and (5) may be resolved by avoidance of chemicals. 

For example, NCEP-ATP uses waist circumference to define obesity, while other expert committees use the body mass index (BMI). The ATP guidelines base glucose intolerance on a fasting glucose while others prefer the results of an oral glucose tolerance test (OGTT). As we are better able to quantify the relationship between these factors and the development of cardiovascular disease, the definition of the syndromes and the individual component values will no doubt continue to be refined. 

Table 18-1 lists the various names given to MCS. The most recent name for MCS is idiopathic environmental intolerances, or IEI (Bornschein et al. 2001). The variety of names reflects the unresolved definition of MCS. A survey of physicians familiar with MCS proposed six characteristics of MCS, as listed in Table 18-2 (Bartha et al. 1999). Table 18-3 lists the most frequent symptoms. MCS symptoms often overlap with symptoms of chronic fatigue syndrome and fibromyalgia (Jason et al. 2000 Nawab et al. 2000). Prevalence of MCS in community surveys and occupational health clinics was found to be 6.3% and 23.0%, respectively (Kreutzer et al. 1999 Kutsogiannis and Davidoff 2001). 

Pharmaceutical packaging, because of the medical risk attached to its products, probably requires a fourth category, possibly termed intolerable defect . This category fulfils the need for the type of fault which, by definition, results in batch rejection and quarantine if one example is found anywhere at any time. It is therefore not necessarily related to any AQL. 

The simplest definition of sustainability refers to the ability of mankind to reach their needs at present and at the same time not compromising the needs and capacity for future generations. Well-established oil palm plantations seem to meet these standards, with an exclusion that these plantations were done at the expense of deforestation. This act is deemed unsustainable, because destruction of pristine forests has permanent effect on the global ecosystem and future sustainability, thus, leading to the conclusion that deforestation is intolerable. Nevertheless, Robinson reviewed that in order for this present generation to meet their needs, agricultural developments are inevitable and naturally for these activities to take place, land is needed (Robinson 2004).
