Chronic dermatitis

For chronic dermatitis, lubricants, emollients, or moisturizers should be applied after bathing. Soap-free cleansers and colloidal oatmeal products also alleviate itching and soothe the skin. 

A provocation of nickel eczema after oral ingestion of nickel has been reported [401, 402], Ingestion of 2.5 mg nickel as the sulphate aggravated chronic dermatitis in 17 of 28 patients [402]. When these patients were put on a low-nickel diet, 9 of them showed improvement.

If the patient s reaction does not subside within a few days, or further spread occurs, the patient should be referred for specialist follow-up and for prescription therapy with stronger topical corticosteroids and possibly oral corticosteroid therapy.

Nevertheless benign monoclonal cutaneous lymphocytic infiltrates are more likely to develop cutaneous lymphoma than polyclonal infiltrates, which usually continue to have a benign clinical course.

The effects of chromium compounds have been studied since World War II. While chromic acid mist was known to produce septal perforations in the nose, chrome holes of the skin, and chronic dermatitis, occupationally caused lung malignancies from certain chromate compounds were a newly discovered epidemiological finding. Exposure to vinyl chloride produced angiosarcoma (cancer of the liver) in workers exposed to the chemical. This wasn t discovered until an epidemiological survey revealed workers in plants that had vinyl chloride in the process had abnormally high incidences of liver cancer. 

Sensitivity to this vehicle is extremely rare, but has been reported in one patient who had positive reactions to all patches in the standard series, where petrolatum was used as vehicle. Patch tests with pure white and yellow soft paraffin were positive (Grimalt and Romaguera 1978). A chronic dermatitis and hyperpigmentation from petrolatum has also been reported (Maibach 1978). 

Eczematous skin lesions are characterized by their chronic evolution and their frequent spread, despite the topical administration of corticosteroids. They are caused, as was shown by Hjorth and Trolle-Lassen (1963), by parabens contained in corticosteroid ointments. Patch tests done with different types of parabens yielded positive responses. Local application of pure corticosteroids, without preservatives, brought about rapid healing. This type of allergy, falsely related to corticosteroids, was particularly studied by Shorr (1968) 1 % of his patients treated for chronic dermatitis suffered from this allergy.
