The administration feels powerless to intervene effectively in treatment-related issues. 

Another LCI-related issue is allocation. This is especially relevant to waste treatment options when waste is recycled or used to generate energy, there is a multi-output process which needs to be dealt with. The waste then is no longer considered a waste, but a resource, and waste treatment is becoming a production process as well. What part of the emissions to allocate to the waste treatment service and what part to the secondary material or other co-product is then open to debate. ISO allows for various options. Whether additives are even visible in such processes is, again, doubtful. 

A related issue in pharmacoeconomics trials is the generalizability of the health care delivery system of the patients in the study. A pharmacoeco-nomic study conducted through health a maintenance organization using its members as subjects may observe less referrals to specialist physicians than would the same clinical study in a different practice setting. This effect may be even more pronounced in multinational clinical trials, in which health care systems, physician education, and patients expectations for treatment differ by country. 

An important related issue is the ideal frequency of maintenance ECT treatments (e.g., weekly, biweekly, monthly). One common clinical strategy is to taper the frequency of acute treatments to once every week for 4 weeks, then every 2 weeks, and then every 3 weeks, as clinically indicated. From a practical perspective, a monthly or bimonthly schedule would be desirable in terms of cost, convenience, risk, and side effects. Unfortunately, few data presently exist to guide clinicians as to the best interval between maintenance treatments, and this determination remains an empirical process with each patient. 

A related issue is that many psychotropics are known to lower the seizure threshold. Therefore increased care is warranted when these agents are used in conjunction with TMS. However, Turnier-Shea and colleagues (213) reviewed their experience with 82 patients on various medication combinations who also received a total of 986 TMS treatments. There were no seizures reported, and the authors suggest that the concomitant use of some antidepressants and TMS should not be prohibited. Here, SF-rTMS may be preferable to HF-rTMS given that most reports of seizures involved the latter approach and because SF-rTMS may induce neuronal inhibition rather than excitation (191, 205). Imaging and EEG data in combination with clinical studies should provide more information about the effectiveness and safety of different frequencies. 

Preliminary results from case reports, case series, and open and increasingly better controlled trials have found TMS to be effective in the treatment of depression. TMS, antidepressants, and ECS appear to induce similar effects in animal behavioral models of depression and changes in brain neurochemical activity. These include changes in the forced swim test, increase in seizure threshold, and effects on b-noradrenergic receptor downregulation. Whereas seizures appear unnecessary to achieve clinical benefit with TMS, it is still unclear what aspects are beneficial. As noted earlier, although this modality is associated with some complications (e.g., headaches and, rarely, inadvertent seizure) relative to ECT and perhaps even pharmacotherapy under certain situations, it appears to be quite safe. Future studies of major depression should explore TMS-related issues such as the following ...

Notwithstanding the commonalities in the engineering analyses of a chemical product s behavior and its manufacturing process, there are fundamental differences between the design of a chemical product and its manufacturing process. For example, chemical process design primarily seeks to identify the lowest cost process. Even process related issues like reliability, controllability, and pollution control ultimately translate into costs that must be minimized. Thus, process design easily lends itself to a mathematical treatment. 

In this volume, we treat AFM of synthetic as well as natural and biological macromolecular materials. The focus is, nevertheless, laid on the materials science and technology aspect, i.e., biological function and biology-related issues are left out. Instead, morphology and structure imaging, surface properties, nanoscale mechanical properties, and practically relevant processes (phase transitions) for typical laboratory and industrial applications constitute the bulk of the treatment.
