Mil STD-882B (11) outlines the general concepts of Systems Safety. Notice 1 (12) appended to that document specifically covers requirements for software systems safety. The Air Force also has a Software Systems Safety Handbook (13). A number of recent articles in Hazafd Prevention (14), the Journal of the Systems Safety Society, expanded on the technical content of this handbook. Most major DOD agencies and contractors involved in the traditional weapons and aerospace programs have internal system safety organizations to plan and implement system safety efforts. Failure Modes and Effects Analysis (FMEA) examines systems, element by element (Systems Safety Society 1997,3-111). The analysis procedure requires the identification of the individual components that make up the system under examination. With the components identified, the modes in which the component failures as well as the effects that that failure has upon the system are determined. A further step to the FMEA procedure is the examination of the risk associated with the failure. The risk, also referred to as criticality, provides the investigators with a method of ranking the hazards in the system and providing a method for prioritization of hazards. [Pg.138]
