The information in this part is an expansion and update based on material previously provided by the author in the System Safety Analysis Handbook (Stephans andTalso 1997). 

Much of the content of this chapter is based on an OSHA pamphlet (U.S. Occupational Safety and Health Department 1993) and the System Safety Analysis Handbook (Stephans and Talso 1997).

While the OSHA rule applies to operations that affect workers within a site where certain highly hazardous chemicals are present at stated levels, the EPA rule was developed to additionally protect the public and the environment from the undesired consequences of explosions and other accidental releases. Much of the information in this chapter comes from DOE-STD-1100 (U.S. Department of Energy 19%) and is enhanced with associated material from the System Safety Analysis Handbook (Stephans and Talso 1997) and other sources as specified.

PSM or RMP audits have multiple objectives. Most important is the assurance an audit brings or the adequacy of implementation of the particular program. Specifically, the audit measures program effectiveness, identifies deficiencies, and verifies correction of previously identified deficiencies. The audit also provides management with a status assessment and may recommend improvements and identify good practices. The material in this section is based on a portion of the content of a Nuclear QA Certification Course taught at Sandia National Laboratories and several companies, the ASQ Nuclear Auditor Training Manual (ASQ 1986), the System Safety Analysis Handbook (Stephans and Talso 1997), and the OSHA Inspection Manual (U.S. Department of Labor 1994). 

The System Safety Society publishes the Journal for System Safety and texts, which include the System Safety Analysis Handbook that has been sold in 38 countries. The Society also honors its own members annually for various achievements and provides a vast networking opportunity for members to solve problems and identify job opportunities. 

It is common now to see the application of the system safety approach, tools, and techniques in more and more industries without using the words system safety.This is evident by the more than 100 techniques described in the System Safety Analysis Handbook.V/hile I am thrilled that the philosophy continues to expand, it is important to understand the basis for which most of the techniques are derived to ensure that they are applied appropriately. 

Warner Talso is the conscience of the System Safety Society and has been for more than ten years. He is the editorial power behind the publication of the first two editions of the System Safety Analysis Handbook. He is a best friend, a confident, and former Army nuclear weapons officer. I ll miss our Saturday breakfast burritos since my wife and I have moved to Nevada.
