NASA was an early user and developer of system safety methods for its complex systems. Most people recognize that space exploration is a risky business. The agency has introduced some concepts that are worth noting. One is risk-informed safety case, intended to determine if something is adequately safe. Another concept addresses the problem of what is adequately safe. NASA uses the term as safe as reasonably practicable. The NASA System Safety Handbook explains the concepts further (note 5). [Pg.523]

Mil STD-882B (11) outlines the general concepts of Systems Safety. Notice 1 (12) appended to that document specifically covers requirements for software systems safety. The Air Force also has a Software Systems Safety Handbook (13). Military Handbook (MIL-HDBK)-516B, Airworthiness Certification, DoD Handbook, delineates the U.S. military aircraft airworthiness certification process. It also contains a list of the typical certification data required for airworthiness certification, which leads to a flight clearance. It includes a section on system safety, a system safety program (SSP), and a software safety program (SwSP). System safety generally provides the safety evaluation of hardware and software for airworthiness flight clearance recommendations. System safety is involved in ensuring that the Aircraft Airworthiness Authority s standards are satisfied for the airworthiness authority involved. [Pg.24]

After such deviations are defined, for each individual point the possible reasons and effects are to be considered. Finally, it can be ascertained whether the available measures are sufficient, or additional ones are required. Important for the efficiency of such an analysis is the experience of the team in charge. The members of this team should consist of experts in production, process engineering, control systems, and relevant authorities such as the TUV.
