Permission Analysts must have the permission of the operators and the supervisors to conduct even the most straightforward tests. While this is part of the analysts preparation, it is important for all involved to know that analysts have that permission. 

Maintenance is not a glamorous procedure however, its importance is second to none. Maintenance procedures are always controversial, since the definition of upkeep varies with the individual interpretation of each maintenance supervisor. The latitude of maintenance ranges from strict planning and execution, inspection and overhaul, accompanied by complete reports and accounting of costs, to the operation of machinery until some failure occurs, and then making the necessary repairs.

Root cause 1 Procedures for shift changeover were inconvenient for use. The prescribed changeover procedure was detailed and elaborate, so it was not used in practice, being seen as too inconvenient for practical purposes. Consequently, one important aspect was omitted. Two fatal cases of occupational exposure to 1,2-dibromoethane were reported by Letz et al. (1984). A worker collapsed shortly after entering a pesticide storage tank containing residues of 1,2-dibromoethane he remained in the tank for 45 minutes. A supervisor attempting to rescue the worker also collapsed and was exposed for 20-30 minutes prior to rescue. Both men died 12 and 64 hours after collapse, respectively. The primary route of exposure was postulated to be dermal, with inhalation also playing a potentially important role. Neither worker had been wearing protective clothing or respirators.

In order to insure that every employee is prepared to work safely, every supervisor has the responsibility to provide a safe work environment with proper equipment and adequate training. Each supervisor also has the responsibility, through personal example and the involvement of all employees, to create a climate in which everyone shares concern for the safety and security of their fellow workers and for the prevention of losses.
