The agreement requires producers for the establishment to control all elements of production to prevent drug residues. In implementing this strategy, FSIS has worked with constituent groups to encourage adoption of producer-driven voluntary food safety initiatives and to coordinate efforts to identify animal production practices that have the potential to eliminate drug residues in edible animal products. 

The importance of a comprehensive specimen identification system throughout the clinical laboratory cannot be understated. Both the College of American Pathologists (CAP) (1) and The Joint Commission (JCAHO) (2) have addressed this important subject with directives that address the pre-analytical, analytical and post-analytical phases of specimen processing. These patient safety initiatives date back to the 1990 s and the formation of the National Patient Safety Foundation (3). These mandates have become the backbone of most laboratories Quality and Safety Programs and offer an excellent foundation for procedures that assure the safe and accurate identification of patient specimens throughout the analysis and reporting of critical laboratory tests. 

Soraya Dhillon is a Foundation Professor and Head of The School of Pharmacy at the University of Hertfordshire. Professor Dhillon has extensive experience in Clinical Pharmacy and Clinical Pharmacokinetics and has held positions in Community and Hospital Pharmacy. She has published widely in the evaluation of clinical pharmacy services and education. She currently holds a nonexecutive role as Chairman of Luton Dunstable Foundation Trust and has a particular interest in driving forward patient safety initiatives.

Since most of the deaths from fires occur in residences, the NFPA proposes the following safety initiatives to improve fire safety (1) increase fire safety education on fire prevention and what to do if a fire occurs (2) install smoke detectors in all homes and check them periodically to ensure they are working properly (3) practice escape plans with the family (4) install residential home sprinklers to prevent fires from spreading once they start (5) develop products for the home that are more fire safe and produce less toxic combustion products (the latter is proposed by the authors) and (6) study the needs of the populations most at risk (the young, the elderly, and the poor) and implement preventive measures. 

This audit led to the formation of two teams, one to tackle locomotive design issues and the other for safety reporting and action. As a result of team activities, many ergonomic actions were implemented. These included management actions to ensure a uniform wagon fleet, autonomous inspection/ repair teams for tracks, and multifunctional teams for safety initiatives.

From the Fig. 4, it s shown that the time to achieve expected safety performance is affected by safety initial level of each factor. The higher the initial safety level is, the less time is used to an expected safety performance after increasing safety input of each factor.

Today I would like to outline the current U.S. perspective on nuclear materials safety. I believe the experiences gained through our past cooperative activities, including those related to nuclear reactor safety, can play an important role in the success of the nuclear materials safety initiative. At the same time, I would like to challenge each of you to work together to help better define what this initiative will become in the future. 

To support this nuclear material safety effort, the DOE has begun to develop a strategic plan that would allow us to allocate resources and work together with you and the international community to design approaches to address nuclear material safety issues. The capabilities and talents currently supporting our cooperative nuclear reactor safety activities, in combination with our growing cooperation within the weapons complex, provide the experience to point the way and provide the basis for successful implementation of a major nuclear materials safety initiative.

What is described here is a reflection of the safety initiatives in a few large, worldwide companies where the safety record truly is superior, meaning outstanding in relation to others in similar businesses. It s my view that if an entity wants to achieve superior safety results, compared to the best in the world, the concepts and procedures set forth here must be adopted. As the term safety is used in this chapter, it encompasses environmental affairs, occupational health and safety, and product safety. 

The board of directors and senior management lead the safety initiative and make clear by what they do that safety is a fundamental within the organization s culture. 

A safety initiative cannot succeed without soundly established and implemented safe work practices. How well that s done is another reflection of... 

It is impossible for superior safety performance to be attained if executive personnel do not display, by their actions, that they intend to achieve it. Management is what management does. What management does establishes the organization s culture. If what management does gives positive impressions, it is more than likely that a safety initiative will succeed. 

While safety professionals may undertake many tasks in their work, the underlying purpose of each task is to have the attendant risks at an acceptable level. Every element of a safety initiative should relate to hazards and the risks that derive from them. 

The goal in the operational mode is to eliminate or control hazards before their potentials are realized and hazards-related incidents occur. Achieving that goal should be undertaken within a continuous improvement process. For every safety initiative in the operation mode, the first consideration should be, Can hazards be ehminated or controlled through redesign of the workplace or work methods ...
