Another unique aspect of rectal cancer is the use of neoadjuvant therapy. Preoperative radiation with or without chemotherapy is given to downstage the tumor prior to surgical resection to improve sphincter preservation by making the surgical procedure easier to perform. The issue of preoperative versus postoperative radiation is a subject of debate and investigation in the United States and will require further data to determine the superiority of one method over the other. [Pg.1352]

At the sensitive extreme are those who experience pain of an acute nature even when the stimulus is only moderate. Those hypersensitive individuals are perhaps those for whom even suggestion may cause pain and who exhibit referred pain in a most striking manner. A recent observation would make it appear likely that hyposensitive, average and hypersensitive individuals are about equal in number. Papper, et al., 14 state that only about one-third of a group of 286 unselected patients complained of severe postoperative pain, one-third had moderate pain, and one-third no pain at all. [Pg.165]

The role of postoperative chemotherapy for gastric carcinoma is still not clearly defined. Numerous prospective, randomized trials have been conducted in the United States and Europe, producing conflicting results. For example, Hermans et al. (14), in a meta-analysis of 123 trials, 11 of which could be analyzed for crude mortality odds, showed no improvement in survival after adjuvant chemotherapy. However, at the 1998 American Society of Clinical Oncology (ASCO) meeting, Earle et al. (15) presented a reanalysis of the literature. Twelve trials met the criteria for inclusion in this metaanalysis. They found a small survival benefit in the group that received adjuvant chemotherapy. Currently, postoperative adjuvant chemotherapy is not recommended. [Pg.257]

Carbachol, being a carbamic ester and not an alcohol is a far less suitable substrate for the esterases. The indications for carbachol are glaucoma, postoperative atonic states of the urinary bladder and the gut as well as supraventricular paroxysmal tachycardia. [Pg.294]

A 60-year-old white man was admitted for kidney transplantation. Immediately after reperfusion and intravenous methylprednisolone 500 mg, he developed severe bradycardia with hypotension and then cardiac arrest. After resuscitation, his clinical state improved quickly, but on the morning of the first postoperative day directly after the intravenous administration of methylprednisolone 250 mg, he had another episode of severe bradycardia, hypotension, and successful cardiopulmonary resuscitation. A third episode occurred 24 hours later after intravenous methylprednisolone 100 mg, again followed by rapid recovery after resuscitation. Two weeks later, during a bout of acute rejection, he was given intravenous methylprednisolone 500 mg, after which he collapsed and no heartbeat or breathing was detectable after cardiopulmonary resuscitation he was transferred to the intensive care unit, where he died a few hours later. [Pg.8]

Rofecoxib is approved for the treatment of acute pain and dysmenorrhea at a dose of 50 mg for up to 5 days. The clinical studies indicate that rofecoxib shows efficacy similar to that produced by the maximum analgesic doses of naproxen and ibuprofen (Ehrich et al., 1999). The pain settings in which rofecoxib has been tested include acute postoperative dental pain, the pain of dysmenorrhea for up to 3 days, and postoperative pain for 5 days following surgical replacement of the knee or hip. In contrast, celecoxib is not approved in the United States for the treatment of acute pain, and it appears to be less effective when given acutely than rofecoxib, ibuprofen, or naproxen. The explanation for the differences between rofecoxib and celecoxib in acute pain is not known. [Pg.129]

Delirium during emergence from sevoflurane anesthesia has often been documented. Four patients, an adult and three children aged 3-8 years, who were able to recount the experience, have been reported (599). They had full recall of postoperative events, were terrified, agitated, and distressed, and hence presented with acute organic mental state dysfunction which was short-lived. Two were disoriented and had paranoid ideation. They were not in any pain or were not distressed by pain if it was present. The authors hypothesized that misperception of environmental stimuli associated with sevoflurane s particular mode of action may have been the underlying cause of this phenomenon. Anxiolytic premedication and effective analgesia did not necessarily prevent the problem. [Pg.693]
