The other related subcutaneous routes are dermojet (by which, drug is projected from a microfine orifice using a high velocity jet) and pellet implantation (which provides sustained release of the drug over weeks and months e.g. testosterone).

Some individuals have claimed real hard-core status by using an animal pellet implant gun and taking one in the glute. Do not try this at home. Six cases of complications loosely related to the use of naltrexone pellet implantation during the highly controversial rapid and ultra-rapid opioid detoxification procedures have been reported (22). These included pulmonary edema, prolonged opioid withdrawal states, drug toxicity, withdrawal from cross-dependence to alcohol and benzodiazepines, aspiration pneumonia, and death. The risk of these controversial procedures and of naltrexone in this novel delivery system are high a robust scientifically validated program of research is needed to justify such treatment packages. 

Estradiol pellets (implants) containing pure crystalline 17/3-estradiol have been available for more than 50 years. They are inserted subcutaneously into the anterior abdominal wall or buttock. Pellets are difficult to remove and may continue to release estradiol for a long time after insertion. Implantation should not be repeated until serum estradiol concentrations have fallen to values similar to those at the midfollicular phase of the menstrual cycle. Estradiol pellets have not gained popularity in the United States. 

For the man with symptomatic hypogonadism (decreased libido, energy loss, and erectile dysfunction), a normal prostate examination, and a normal serum prostate-specific antigen, testosterone can be considered as adjunctive therapy. Testosterone patches (2.5-5 mg daily), intramuscular injections (10 to 400 mg every 2 to 4 weeks), 1% topical gel, pellet implants (150 50 mg every 3-6 months), and a buccal formulation are available (see Table 88-6). Kaku et al. (55) reported that acute in vivo or in vitro morphine treatment inhibited Ca uptake into mouse brain synaptosomes, an effect which disappeared with tolerance development. A series of experiments have been conducted in this laboratory in order to examine opiate effects on Ca fluxes in synaptosomes. Morphine caused dose-dependent decreases in synaptosomal Ca uptake at low K+ concentrations (in the presence of ATP and Mg2+) after in vitro or acute in vivo treatment (58). This effect was naloxone reversible and stereospecific, with levorphanol also causing inhibition but not dextrorphan. Uptake was apparently inhibited in a noncompetitive fashion. In contrast, mice made tolerant by morphine pellet implantation showed progressive increases in ca uptake with increasing degrees of tolerance that had developed, p-endorphin similarly inhibited Ca uptake into synaptosomes after in vitro treatment or after i.c.v. injection (59).

Fluprednisolone has been shown to exist in seven different solid phases, of which six were crystalline and one was amorphous [107]. Of the crystalline phases, three were anhydrous, two were monohydrates, and one was a tert-butylamine solvate. The in vitro dissolution rates of the six crystalline phases of fluprednisolone were determined and compared with in vivo dissolution rates derived from pellet implants in rats [108]. The agreement between the in vitro and in vivo dissolution rates was found to be quite good, but the correlation with animal weight loss and adrenal gland atrophy was only fair. These results can be interpreted to indicate that, for fluprednisolone, differences in dissolution rates of the drug did not lead to measurable biological differences.
