Do not assume that such things could not happen in your company (unless you have spent some time in the relief-valve workshop). All relief valves should be tested and inspected regularly. Critical document reviewis done by the PLL team for documents provided by the government, including reports of unusual occurrences or events, safety reviews (including near-miss advisories), reports of nonconformance or non-compliance, reports of test results, audit surveillance inspection reports, daily and weekly operating reports, and campaign reports. The purpose of these reviews is to identity lessons that will be added to the PLL database, update the database as needed, introduce appropriate topics tor discussion at workshops and, if needed, recommend direct actions to secure further information. 

When the PLL program began in 1994-1995, the major source of issues was the review of documents (event reports, end-of-campaign reports, inspection reports, and so on.). Now most of the information comes from the facilitated workshops run by the PLL program, which allow input and peer review by multiple program personnel with expertise in the subjects under discussion. The initiators of the information (subjects) are primarily the chemical demilitarization sites, but some issues come from other program participants. Plant items that have been repaired in the workshop should be inspected and tested to ensure, as far as possible, their full return to serviceability. Before a works painting or metal spraying contract for steelwork is awarded, the workshops concerned should be inspected by qualified personnel. It is essential that some check is made to ensure that the necessary facilities and equipment for carrying out all the terms of the order or specification are available. Cleanliness of the painting areas and order in the paint store usually indicate the seriousness with which works personnel take protective-coating operations. 

Adequate lighting levels to achieve manufacture and joints of fabricated elements and inspection of the finished product should be provided in workshops. The appropriate Factory Acts in force regarding lighting levels of illumination and glare must be met. A level of illumination of 500 lumens/m falling on the work surface should be suitable for most operations.
