Unusual occurrences

Routine work typically includes monitoring and reporting load changes, unusual occurrences, machinery failure, steam leaks, and the like. In addition, boiler plant personnel carry out a number of other specific functions, including those tasks described in the following sections. 

The unusual occurrence of ( " U/ U) ratios less than one were observed in waters from the weathered zone around the Koongarra ore deposit (Yanase et al. 1995). While this could be due to prior leaching and depletion of from the U that is now released by weathering, it was suggested that this reflects implantation of recoil into phases more resistant to weathering during undersaturated conditions in this zone. 

Lawrencium - the atomic number is 103 and the chemical symbol is Lr. The original chemical symbol was proposed as Lw but it was changed because W is an unusual occurrence in many languages and it is a cumbersome spoken word. The name derives from the American physicist Ernest O. Lawrence , who developed the cyclotron. Credit for the first synthesis of this element in 1971 is given jointly to American chemists from the University of California laboratory in Berkeley, California under Albert Ghiorso and the Russian scientific team at the JINR (Joint Institute for Nuclear Reactions) lab in Dubna, Russia under Georgi N. Flerov, after a series of preliminary papers presented over a decade. The longest half-life associated with this unstable element is 3.6 hour Lr.

The objective of the process hazard analysis is to identify possible unusual occurrences in the individual systems of the facility or system and to anticipate the possible consequences resulting from the occurrences. Where these occurrences are deemed not to be adequate a recommendation for their improvement is provided.

There are a few exceptions where the DDAs are capable of penetrating coastal waters. For instance, Kimor et al. (1978) reported an unusual occurrence of Richelia associated with Hemiaulus membranaceus off the coast of California and one report of Richelia associated with H. hauckii and H. membranaceus in waters off Hawaii (Heinbokel, 1986). Recently, White et al. (2007) observed high abundances (100 L ) of the Rhizosoknia-Richelia symbioses in the Gulf of California. Hemiaulus-Richelia also occurs at Carrie Bow Cay, Belize (Villareal, 1994), and off the coast of Texas (Villareal pers. comm.).

In 1971/ Herbst et al. (93) reported the unusual occurrence of vaginal clear cell adenocarcinoma in eight young women. The precipitating factor appeared to be the fact that their mothers had been treated with diethylstilbestrol (DES) in order to prevent spontaneous abortion and premature delivery in what were deemed to be high-risk pregnancies. Estimates place the incidence of clear cell adenocarcinoma of the vagina at 1.5 per 1000 women who were exposed in utero to DES (94). 

The Chinese Whispers effect could have serious consequences in a pharmacy environment. In a worse case scenario there could be seven or eight possible interactions involved. Consider the scenario below, a not unusual occurrence in a community pharmacy ...

NOV-94 JACADS (Unusual Occurrence) ECR Detonation of rocket on fuze shear caused agent migration to observation corridor. All agent vapor contained under engineering controls and exhausted through the MDB charcoal filter units.
