Support the airway. This may require intubation. All cyanide exposure patients should get 100% oxygen and support of ventilation if needed. Obtain IV access. Use a cyanide antidote kit if you are equipped (Figure 2.11). The cyanide antidote kit is itself toxic, and you should have special training if you are to use it. Most EMS units do not carry the kit, and most hospitals stock only one or two kits. There is an easier and safer treatment being used in Europe (IV hydroxycobalamin) and it has recently been accepted by the FDA for use in the United States. 

There are three general types of radiopharmaceuticals elemental radionucHdes or simple compounds, radionucHde complexes, and radiolabeled biologically active molecules. Among the first type are radionucHdes in their elemental form such as Kr and Xe or Xe, and simple aqueous radionucHde solutions such as or I-iodide, Tl-thaUous chloride, Rb-mbidium(I) chloride [14391-63-0] Sr-strontium(II) chloride, and Tc-pertechnetate. These radiopharmaceuticals are either used as obtained from the manufacturer in a unit dose, ie, one dose for one patient, or dispensed at the hospital from a stock solution that is obtained as needed from a chromatographic generator provided by the manufacturer. 

Dextrose used in PN compounding typically is provided as a 70% stock solution (70 g/100 mL), although some institutions use a 50% stock solution. The final dextrose concentration in the PN solution typically should not exceed 35%. Hydrous dextrose provides 3.4 kcal/g (14.2 kj/g). A dextrose infusion rate of 2 mg/kg per minute should be sufficient to suppress gluconeogenesis and prevent protein breakdown in adults.5 Continuous dextrose infusion rate in adult patients generally should not exceed 4 to 5 mg/kg per minute in most hospitalized patients.6,7...

Sorbitol is a polyhydric alcohol, with a high caloric content. In a survey of 129 oral liquid dosage forms stocked at a large university teaching hospital, 42% contained sorbitol [66]. The sorbitol concentration in the identified products varied from 3.5 to 72% w/v (0.175-3.6 g/mL). 

Calendering involves passing unformed or extruded rubber through a set or sets of rolls to form sheets or rolls of rubber product. The thickness of the material is controlled by the space between the rolls. The calender may also produce patterns, double the product thickness by combining sheets, or add a sheet of rubber to a textile material. The temperature of the calender rolls is controlled by water and steam. Products produced by this process include hospital sheeting and sheet stock for other product fabrication. 

I was appointed to the position of Bed Manager within a large National Health Service hospital with 450 beds Just over two years ago. This was a recently created, nursing-grade post. The sale of tryptophan as dietary supplements for man is now illegal. Dietary supplements to animal stock feed is OK. Tryptophan is available to hospitals for use in critical situations. Tryptophan is available as a prescription drug. But it is not available in the health food stores and so cannot be explored by the lay researcher. The world of inquiring into the action on normals, schizophrenics, alcoholics, people who are overweight, people who are depressed, is denied both to the private individual and to the clinical researcher. There are commercially available drugs, all approved, that can play the same role. Within four days of the announced ban of tryptophan (after the problem had been resolved and corrected) a broad promotion of Prozac (an antidepressant similar in action to Tryptophan) appeared in Newsweek (March 26, 1990). Prozac is still widely promoted. Tryptophan is still not available to the private individual. Both can play the role of being an effective sedative. 

The fear of opioids falling into the wrong hands has prompted some pharmacies not to stock narcotic analgesics, and some hospitals have limited the use of opioids to cancer patients only. There have even been reports of patients keeping their prescriptions secret out of fear of having their pills stolen.

When the incident was revealed more than 3 years later (Suskind, 2006), it was also reported that at the time of the alert city hospitals were wrestling with the issue of how to treat anyone exposed to cyanide and sought to increase their stocks of medical antidotes to cyanide and other toxic substances, preparing for any potential mass triage (Baker Rashbaum, 2006). In other words, when federal and local authorities feared a chemical attack in New York City, the news media did not get wind of it and, as a result. New Yorkers were neither aware of the threat nor of the health community s rush to prepare for the worst.

It may be restricted to what a third party payer will reimburse, or to the range of formulations stocked in a hospital (and chosen by a local drugs and therapeutics committee, which aU hospitals or groups of hospitals should have), or the range agreed by a partnership of general practitioners or primary care health centre.
