General practitioners

In countries with an existing (social) health insurance system, it is usually rather simple do receive a close-to-reality estimate of the provider Costs-of-Illness. The insurance pays the bills of general practitioners, specialists, hospitals, pharmacies, laboratories, etc. so that the total costs per patient can easily be determined. However, in some countries we cannot receive this data, and sometimes confidentiality regulations do not permit the transfer of insurance data, so that, for instance, provider costs of difference phases of HIV/AIDS can be calculated. In this case, a sample of patient files has to be analyzed with permission of the patients so that the provider costs can be recorded.

A further factor concerns the milieu in which the anxiety disorders are encountered. These are essentially detected, diagnosed and treated by primary-care practitioners. In the UK, for instance, less than 10% of such cases are referred to specialists, although the figure tends to be higher in the USA and continental Europe. General practitioners often make syndromal or even symptomatic assessments rather than diagnosing disorders, so it may be difficult to extrapolate across countries.

Where Phase II, III and IV studies are conducted in a hospital or contract research environment, it is strongly advisable to inform the subject s general practitioner (GP) in writing of the nature of the study and to obtain the GP s agreement, preferably in writing. This is essential for Phase 1 studies in non-patient volunteers, and it is routine practice in all Phase 1 clinical pharmacology units.
