Attention is drawn to the Guidelines for Company Sponsored Safety Assessment of Marketed Medicines (SAMM) which have been produced jointly by the ABPI, the British Medical Association, the Committee on Safety of Medicines, the Medicines and Healthcare products Regulatory Agency and the Royal College of General Practitioners. It is good practice to reviewa project on completion and record the reasons for departure between planned and actual performance. Where lessons can be learned, or opportunities exploited, they should be incorporated into project management guidelines. Some companies hold post project sessions with their contractors to explore better ways of handling particular issues, especially when there is an expectation of additional shared activities. 

ISO 9000. The ISO 9000 series of standards was first issued in 1987 and then updated in 1994. The ISO 9000 standard describes the selection criteria for the four standards, ISO 9001—9004. ISO 9001 is composed of 20 items covering manufactured products from design and development in R D to commercial production and after-sale service or technical support. ISO 9002 has 19 of the items in 9001 describing the requirements of a quahty system for the manufacture of an item, such as chemicals, to a specification only the requirement relating to product development is omitted. Chemical companies seldom certify to ISO 9003, which describes the quahty system for a laboratory involved in final inspection and test. Finally, ISO 9004 presents guidelines for total quahty management. 

Table A.4, taken from the CCPS Guidelines for Chemical Reactivity Evaluation and Application to Process Design, shows the questions which need to be asked regarding the safety of the proposed reaction, the data required to answer those questions and some selected methods of investigation. The experimental analysis is extremely specialized, and companies should consider outsourcing the tests if they do not have specialists in this area.

The Center for Chemical Process Safety (CCPS) identified the need for a publication dealing with process safety issues unique to batch reaction systems. Remember that the CCPS definitions may not fit all companies or situations however, they provide a useful set of guidelines for considering how a management system like PSM can work within your company. Some companies have adopted these guidelines very literally and use a highly documented, checklist approach. Others follow them conceptually, in a less structured manner. 

To meet this requirement, a company must define an incident in specific terms for their facility. This includes an operational definition that indicates the number of pounds of the substance used in a particular process that would qualify as a catastrophic event . Defining an incident in site-specific terms also includes defining the terms "could reasonably have resulted in . Appendix C of the regulation provides guidelines for clarifying this point. It includes definitions of "near misses in which a catastrophic failure occurred, but a chemical release did not occur. Clear guidelines should be established that provide the employee with a quantifiable means of defining those incidents that require a violation report. 

The particular facility under review is a unit producing a reactive material from a high-burning velocity hydrocarbon. The company has established internal guidelines for evaluating occupied buildings that are exposed to identified explosion hazards. 

In this book I will share with you the process by which I came to this conclusion and the scientific evidence on which it is based.
