Personal preference is important, but not as important as selecting a device that accomplishes the intended treatment plan. It is important to consult all health care persormel that will be accessing or maintaining the device to be placed. Nurses may be unfamiliar with extremity ports and have difficulty accessing them. This would lead to incomplete treatments. As long as the device will satisfy the needs of therapy and maintenance persormel, the patienfs preferences can be accommodated. Ports will allow patients to return to normal activities and are well suited for the active patient. Some patients are needle-phobic and would prefer avoiding any needles. In this situation, an external device maybe preferable. [Pg.135]

Although ports can be placed within the forearm, in our practice, they are exclusively placed in the upper arm (Pearl et al. 1991 Kahn et al. 1992). Venous access is performed in the mid-portion of the upper arm so that there is adequate room to create the pocket and not interfere with elbow motion. Many patients prefer the extremity port for cosmetic reasons where the port can be covered by a short sleeve. Following insertion of the 0.018-in. guidewire, a small 4-F transition dilator is placed followed by the placement of a hemostatic sheath large enough to accommodate the catheter (typically 6 F). Selecting an approach Off-flavors are typically due to volatile compounds present at extremely low levels. (Flavor is sensed more by the olfactory system than the tongue, which senses only 5 flavors, sweet, sour, bitter, salty, and umami). GC is ideal for detecting low levels of volatile components. In this case, headspace GC will allow you to treat the plastic directly. Since the off-flavor is suspected to be derived from the polypropylene packaging material, you decide to compare different samples ( good vs. bad ) of the material using headspace GC with both a flame ionization detector (FID) and a sniff port. There are two basic methods used to address these effects back pressure and the use of curved carriers. Back pressure consists of increasing the pressure applied to the wafer via a port behind the wafer plate. The use of back pressure is preferred since it is readily adjusted to compensate for a broad spectrum of issues. Adequate performance is attainable in most cases. However, in cases of extreme film stress, it is often necessary to use a curved carrier. Such carriers have a wafer plate with a small amount of curvature built into them.
