There are many instances when a family member (collateral) will seek help for a problem before the person with the drug problem does. Often, however, the family member will not be forthcoming about the drug problem, or it will not be otherwise obvious that drug abuse is indirectly involved. Excellent examples of these types of situations include the family members seeking help as victims of domestic violence or parental neglect, or are injured in accidents. 

Gun Ownership Tied to Higher Risk for Women s Murder, Suicide. American Medical News, vol. 40, April 21, 1997, p. 18. Reports a study that identified risk factors for death of women by murder or suicide. Guns are the most common cause of death, and mental illness, substance abuse, and domestic violence are all important factors. The article recommends encouraging people to store guns safely or to remove them from the household and supports laws that forbid gun purchase by convicted batterers. 

Reibstein, Larry, and John Engen. One Strike and You re Out. Newsweek, vol. 128, December 23, 1996, p. 53. Explains that a 1996 law, intended to cut down domestic violence by banning gun ownership by persons convicted of even relatively minor violent acts, may result in many law enforcement officers being no longer able to carry guns. After heart disease and cancer, alcohol-related disorders are considered the third most important health problem in the United States, estimated to account for at least one fourth of all hospitalizations in this country. Almost 50% of those who suffer from alcohol dependence also abuse other legal and illicit drugs (see Appendix D). Alcohol is involved in 25% to 35% of all suicides and 50% to 70% of all homicides it also figures prominently in accidental deaths and domestic violence (388, 389). Furthermore, causes of death resulting from excessive alcohol use include the following ... 

Over 50% of Americans who currently drink report having a close relative who abuses alcohol 25% are children of an alcoholic parent. Alcohol abuse can devastate families by causing separation, divorce, and domestic violence, as well child abuse and neglect. Six million children live with an alcohol-abusing parent, and this can result in problems at school such as low attendance, academic difficulties, attention deficit disorders, and behavioral problems. Each year in America, more than 100,000 people die from alcohol-related accidents (cars, falls, fires, drownings, burns), cancer, liver disease, and stroke. 

Amphetamine abusers and addicts become preoccupied with when and where they will be able to get their next dose. Relationships with family and friends frequently deteriorate as the drug takes center stage in the addict s life. Money problems may began to surface as the addict funds his growing habit. Substance abuse also contributes to crime, domestic violence, sexual assault, drop-out rates, unemployment, and homelessness. It is also a factor in the spread of sexually transmitted diseases (STDs) and unwanted pregnancy. 

This review generated responses from other authorities in the field. Some commented that the conclusions may be premature, given the age of the subjects, and drew attention to several studies that have shown subtle but consistent deficits in cognitive and attentional processes in 6-and 7-year-old children (302). These effects may become more prominent as development continues and may persist into adulthood. Others criticized the attempt to isolate cocaine exposure from all other associated risk factors from a public health perspective, prenatal cocaine exposure clusters with other risk factors, such as poor caregiving, child maltreatment, domestic violence, and prenatal exposure to other substances (303). Within the EMLA arena, safety professionals should be aware of the proposed Balancing Act of 2009, which would amend and increase the scope of the EMLA to include 12 weeks of paid leave for family issues, 7 days of paid sick leave, parental involvement leave, domestic violence leave, and other expansions of the current EMLA. Under the proposed Healthy Family Act, companies and organizations would be required to provide 1 week of paid sick or health-related leave each year.

In the past years, an increase has been detected in using chemical agents in aggressions (Fig. 4.53) involving domestic violence. 

Although a lot of the focus is on patients who are drunk, on drugs, mentally iU, or gang members, there are other potential sources of violence as weU. Domestic violence situations involving staff members can spill onto the hospital campus. Disgruntled workers or former employees can cause trouble. The security department has to be prepared for all of these scenarios. 

It is important to discuss how faculty and staff manbers should look out for one another. Awareness is a key. Individuals should inform each other about people or situations that may pose a threat or risk of harm. This can include letting others know of domestic violence issues that could intrude upou the workplace. This should also include individuals who may be disgruntled, aggressive, unstable, and the like. Potential situations should be discussed so that individuals in the work environment know what things to look for and what actions to take to protect themselves, others, and summon appropriate help if an incident occurs.
