Eight cases of nervous system diseases suspected to be either recurrent disseminated encephalitis or multiple sclerosis after hepatitis B immunization have been reported (21). Symptoms started in four cases at 4—14 days after vaccination, and in the other four cases 42-70 days after vaccination. There was a family history of multiple sclerosis in two patients, one of whom had had symptoms compatible with optic neuritis before immunization. In a third patient, there was a history of an episode compatible with Lhermitte s sign. The authors concluded that the risk of demyelinating diseases is unknown. There are many areas of application of MRI and MRS in pharmaceutical research, including arthritis, cancer, cardiovascular diseases, central nervous system diseases, renal diseases, respiratory tract diseases, and various toxicological applications. Nervous system Fluorouracil can cause acute nervous system toxicity. Acute cerebellar syndrome affects up to 5% of patients and is usually self-limiting after withdrawal. It can occur within weeks to months of starting fluorouracil and presents with ataxia, nystagmus, and dysarthria [77, 78 ]. An encephalopathy can occur rarely and is often associated with markedly raised ammonia concentrations in the absence of underlying liver disease. Ischemic stroke has also been reported and the risk appears to be increased when fluorouracil is combined with cisplatin [60, 79 ]. Other rare adverse reactions include oculomotor disturbances, focal dystonia, parkinsonian syndrome, peripheral neuropathy, and seizures [80 ]. Dihydropyrimidine dehydrogenase deficiency also increases the risk of nervous system toxicity [81" ]. [Pg.738]

Magnetic resonance spectroscopy (MRS) measures the levels of different metabolites in body tissues, usually in the evaluation of nervous system disorders. Concentrations of metabolites such as N-acetyl aspartate, choline, creatine, and lactate in brain tissue can be examined. Information on levels of metabolites is usefiil in determining and dis nosing specific metabolic disorders such as Ganavan s disease, creatine deficiency, and untreated bacterial brain abscess. MRS has also been usefiil in the differentiation of high-grade from low-grade brain tumors. [Pg.1150]
