Tolbutamide is one of the most widely used antidiabetic agents. Its action is preferably connected with stimulatory action of j3-cells in the pancreas, which results in intensive insulin secretion. PURPOSE AND RATIONALE Anti-insulin antibodies (ALA) develop in the serum of many patients who are receiving insulin treatment (Berson et al 1956). In addition, insulin auto antibodies (IAA) are detected in insulin dependent diabetes melhtus (IDDM) or Type I Diabetes before insulin therapy (Palmer et al 1986).

A 69-year-old woman with diabetes melhtus and heart failure repeatedly had unusual subtherapeutic plasma digoxin concentrations (56). When acarbose was withdrawn the plasma digoxin concentration rose. 

Epilepsy and osteoporosis are very common and frequently overlap. Nevertheless, the prevalence of low bone density appears to be disproportionately higher in patients with epilepsy, and patients with epilepsy have an excessive risk of fractures. A meta-analysis of 94 cohort studies and 72 case-control studies has shown that anticonvulsant treatment is highly associated with fractures (relative risk over 2) (117). Other risk factors were low body weight, weight loss, physical inactivity, consumption of corticosteroids, primary hjrperparathjroidism, type 1 diabetes melhtus, anorexia nervosa, gastrectomy, pernicious anemia, and age over 70 years.

Diazoxide can cause hyperglycemia and diabetes melhtus and has been used to treat hyperinsuhnism in infancy, although its use may be hazardous (2,3). 

A 70-year-old woman with a history of diabetes melhtus and renal insufficiency rapidly developed a severe anemia (with a hemoglobin concentration of 3.5 mmol/1). She had been treated with recombinant erythropoietin (4000 lU/ week) for 6 months with an initial favorable response (hemoglobin concentration between 6.8 and 7.5 mmol/1) (82). Her bone marrow showed pure red cell aplasia with early termination of maturation. The dose of recombinant erythropoietin was increased to 8000 lU/week. She required 3 units of packed red cells per month. 

In untreated women, the main risk factors for endometrial carcinoma are age, obesity, nulliparity, late menopause (and possibly early menarche), the Stein-Leventhal syndrome, exposure to exogenous estrogens, radiation, and certain systemic diseases, including diabetes melhtus, hypertension, hypothyroidism, and arthritis (SED-14, 1451) (81). Certain of these risk factors indicate that an altered endocrine state with increased estrogen stimulation is a predisposing cause, and one might thus in theory expect estrogen treatment (and notably hormonal replacement therapy) to increase the risk (SEDA-22, 466). 

Ibuprofen (150 mg, 3 doses) for arthralgias was associated with hypoglycemia in a 72-year-old man who was taking glibenclamide 2.5 mg/day for type 2 diabetes melhtus after the last dose he lost consciousness, and his blood glucose concentration was under 2.2 mmol/1 (33).
