Introduced into ophthalmic practice in 1942, polyvinyl alcohol (PVA) is a water-soluble viscosity enhancer with both hydrophilic and hydrophobic sites. A common concentration used in ophthalmic preparations is 1.4%. PVA is useful in the treatment of corneal epithelial erosion and dry eye syndromes because it is nonirritating to the eye and actually appears to facilitate healing of abraded epithelium. It is used also to increase the residence time of drugs in the tears, aiding ocular absorption. [Pg.32]

An elderly man underwent vitreoretinal surgery for retinal detachment. Perfluorodecahn was used to repair the retina and was left in situ for 8 weeks and removed via the pars plana. One month later he developed a non-healing corneal epithelial defect associated with limbitis. Perfluorodecahn was found under the superior conjunctiva. A biopsy showed vacuoles in the conjunctival stroma surrounded by inflammatory cells. On surgical removal of the perfluorodecahn from the subconjunctival space, the epithelial defect healed. [Pg.2654]

Nutritional vitamin A deficiency causes xerophthalmia, a progressive disease characterized by night blindness, xerosis (dryness), and keratomalacia (comeal thinning), which may lead to perforation xerophthalmia may be reversed with vitamin A therapy. However, rapid, irreversible blindness ensues once the cornea perforates. Vitamin A also is involved in epithelial differentiation and may have some role in corneal epithelial wound healing. Tissue repair is a very important process in eye irritation, particularly corneal lesions, but has rarely been studied. Epithelial migration and proliferation are the primary events occurring after epithelial injury. It has been shown that fibronectin stimulates epithelial cell migration, whereas hyaluronic acid is more active on proliferation (Salonen et aL, 1991 Inoue and Katakami, 1993). Growth factors (EGF, FGF, etc.) have also been shown to play a role in corneal epithelial and endothelial wound healing (Dabin and Courtois, 1991 Raphael et al. 1993 Hoppenreijs et al., 1994).
