Work permits entry

Readings acceptable for entry shall be recorded on the Safe Work Permit and sh assure that the oxygen content is 21 percent plus or minus 0.5 percent. [Pg.2338]

Safety procedures for lockout, tank entry, hot work permits, and excavation written First aid and medical assistance available First-aid kits, blankets, stretchers, antidotes, and resusci-tators on hand... [Pg.331]

Unit checkout Check that required mechanical work has been completed, tags and blinds pulled, and temporary piping disconnected. Plant supervision must certify completion of work. Cancel all entry and work permits. Utility system has been commissioned. Check blind list and inspect lines close bleed, drain and sample valves. [Pg.333]

The hazards of management-controlled nonroutine work in the process areas must be communicated to affected individuals. The work permit prescribes the procedures that must be followed to get the permit. Work authorization procedures specify lockout/tagout, line breaking., confined space entry, and hot work authorization through clear steps leading to job completion, closure, and return to normal. [Pg.73]

Once a vessel has been connected up to any process or service line, the full permit-to-work and entry procedure should be followed. In the present case, this should have started eight weeks before the incident. And the nitrogen line should have been disconnected or slip-plated where it entered the vessel. [Pg.253]

Finally, PMCD work permits must be issued for all entries into chemical agent hazard areas and areas designated Permit Required Confined Spaces. Work permits, which must identify the individual and the work to be performed, are reviewed by operations maintenance and safety personnel and approved by the shift manager. Special monitoring may be required, and records of entries and monitoring must be kept for 30 years beyond the last day of employment or the closure date of the facility. [Pg.42]

Many of these ignition sources can be controlled through effective housekeeping, equipment maintenance, hot work permit, and general safety programs. Only some of the ignition sources are discussed in greater detail in this entry. [Pg.1115]

The hazards associated with operations that involve more than one group can be reduced by the use of a permit system. O Driscoll has described the permit systems used in chemical operations many of these (hot work tank entry line breaking disposal operations excavations operation of material handling equipment and vehicles sprinkler-valve closing permit) are of interest in low-temperature plants. They are designed to permit the safe conduct of maintenance and repair functions. The use of written permits with copies to all groups concerned tends to increase the safety of potentially hazardous operations. Also, with use of written instructions, there is less chance for a misunderstanding. [Pg.107]

Lactam 243 was thus proposed to be a versatile intermediate which could possibly permit entry to echitamine or akuammiline type scaffolds. No further work was described by this group toward these ends. This work, however, can be considered as the first late stage construction of the C6—C7 bond to form the indoline quaternary center of the akuammiline core. [Pg.217]

Will the work require a hot work permit a confined entry permit,... [Pg.163]

Signatures are common for confined space entry permits, hot work permits, lockout/tagout procedures, and other safety procedures. Before restarting a process, a signature of the person in charge will mean he approves and accepts any repairs or changes. [Pg.452]

The format of permit-to-work documents will be determined by the particular type of work being undertaken, but all such documents should cover the eight points listed above. Different types of permit will be needed to cover general work, hot work, vessel entry, excavations and high-voltage electrical work. For the scheme to work it is essential that the procedure is properly and fully followed. The responsibility for ensuring this rests with the employer, or the person he appoints to oversee the work. The circumstances under which a permit-to-work scheme is to be used must be fully understood as must the responsibility of each of the participants. This applies to contractors as well as employees and they must be fully instructed in its operation before being allowed to start work. [Pg.696]

Explain the interface between confined space entry and lockout/ tagout, respiratory protection, and hot work permits. [Pg.323]

Are all of the applicable Work Permit Procedures (Confined Space Entry, Lock Out/Tag Out, Hot Work, High Work, etc.) in piace Have the Operating, Maintenance and Supervisory personnel been properly trained on the Work Permit Procedures ... [Pg.115]

The equipment provided for use by backup personnel should be covered by the maintenance manual and work permits. Such equipment includes the protective equipment necessary for entry and the gear required for rescue of personnel from the confined space. The latter includes chain falls, lifelines, and harnesses. These devices must be securely fastened outside the space. The best choice will depend on the shape of the access port(s). In the case of a closed vessel provided with one or more manways, for example, a wrist harness may be preferable to a full-body harness. The latter may make it very difficult to remove a person who is badly injured or unconscious. [Pg.224]

The advantages of having a properly trained attendant are significant. They can review the permit and ensure it is filled out properly. They can review the tasks to be performed and PPE required for the job. They will know that a welder will need a Hot Work Permit in addition to the Confined Space Entry permit, as well as a fire watch and proper fire-suppression equipment. The attendant can also deny access to anyone who does not meet the aiteria set forth on the permit, does not have proper PPE or training, is not listed on the entry permit or if there is an atmospheric condition that has changed the classification of (he space. [Pg.34]

In some parts of a controlled area, compliance with the relevant limits can be achieved only by limiting the time spent there or by using special protective equipment. The definition of different zones within a controlled area, on the basis of dose rates or levels of loose contamination, should be considered. Some zones will necessitate setting conditions for restricted entry and special entry. Administrative control of entry into these zones can be effected by means of local rules or radiation work permits (see paras 3.39-3.47). [Pg.16]

Separate permit forms may be required for different tasks such as hot work and entry into confined spaces and therefore different information and records are likely to be required. [Pg.164]

Determine whether a permit, such as a confined space entry permit or hot work permit, is necessary for craft labor protection. [Pg.25]

Systems of work often operate under the control of a permit system, known as a permit-to-work. Because of the administration needed to run them, permit systems are relatively uncommon in construction. They are more common in industries where the hazards are associated with high risk levels and their use is well established in many organisations. They require certain precautions to be taken in order to control them. Examples include hot work permits, electrical lock-out systems and vessel or other confined space entry systems. [Pg.77]

Where there may be a hazardous atmosphere or deficiency of oxygen in the area where excavation is to take place, atmospheric tests must be carried out before work starts and if necessary during the course of the work. If the excavation becomes a confined space as defined above, a CS permit will be required and must be issued before entry into the excavation. A hot work permit may also be required in some cases. [Pg.82]
