A company s PSM initiative deserves some special treatment to attract the initial attention that will help give it momentum. Depending on what is already in place, a special edition of the company newsletter could be produced, including a letter from the CEO and a copy of the PSM vision statement. Beyond the ATS program, the DOE is looking at several new initiatives to work on -with industry. One, Vision 21, aims to virtually eliminate environmental concerns associated with coal and fossil systems while achieving 60 percent efficiency for coal-based plants, 75 percent efficiency for gas-based plants, and 85 percent for coproduction facilities. Two additional fossil cycles have been proposed that can achieve 60 percent efficiency. One incorporates a gasifier and solid oxide fuel into a combined cycle the other adds a pyrolyzer with a pressurized fluidized bed combustor. Also under consideration is the development of a flexible midsize gas turbine. This initiative would reduce the gap between the utility-size turbines and industrial turbines that occurred during the DOE ATS program. 

Adverse reactions include transitory stinging on initial instillation, blurring of vision, mydriasis, increased redness, irritation, discomfort, and increased IOP. Systemic adverse reactions include headache, browache, palpitations, tachycardia, arrhythmias, hypertension, myocardial infarction, and stroke. 

Be aware of, anticipate and avoid the Normal Chemistiy Education reflex, i.e. the introduction into design and teaching of more chemical concepts than are needed for the chosen curriculum emphasis. Alternatively, deal in time with any difficulties related to the dominant school chemistiy curriculum at all stages of development, starting with the initial vision and the first design of the curriculum. 

The Physiome Project will undoubtedly benefit from lessons learned during the progress of the Genome Project, in particular, that big visions and small steps (at least initially) are not necessarily a contradiction. It will, however, have to develop a completely different approach to problem solving than that used for the Genome Project, as neither the total dimension of the task (there are only 23 human chromosome pairs) nor the size of the smallest component that needs investigating (DNA bases) can be defined at the outset of the Physiome Project.

Severe, acute respiratory acidosis produces a variety of neurologic abnormalities. Initially these include headache, blurred vision, restlessness, and anxiety. These may progress to tremors, asterixis, somnolence, and/or delirium. If untreated, terminal manifestations include peripheral vasodilation leading to hypotension and cardiac arrhythmias. Chronic respiratory acidosis is typically associated with cor pulmonale and peripheral edema.

Select azole antifungals (e.g., itraconazole, voriconazole, and posaconazole) and the echinocandins are available for IA treatment. For initial therapy of IA, voriconazole had higher response and survival rates than c-AMB.102 An advantage of voriconazole is its 96% oral bioavailability, making use of this oral drug an attractive and less expensive alternative. The dose of voriconazole was 6 mg/kg IV every 12 hours for two doses, followed by 4 mg/kg IV every 12 hours for at least 7 days, at which time oral voriconazole 200 mg every 12 hours could be administered. Common toxicities reported with voriconazole include infusion-related, transient visual disturbances (i.e., blurred vision, altered color perception, photophobia, and visual hallucinations), skin reactions (i.e., rash, pruritus, and photosensitivity), elevations in hepatic transaminases and alkaline phosphatase, nausea, and headache.102 In addition, voriconazole increases the serum concentrations of medications cleared by cytochrome P-450 2C9, 2C19, and 3A4 (e.g., cyclophosphamide and calcineurin inhibitors) concomitant voriconazole-sirolimus should be avoided.103...

Increases in the concentration of calcium in the cytosol provides a signal that can initiate muscle contraction, vision, and other signaling pathways. The response depends on the cell type. In muscle, a transient rise in the cytosolic calcium levels (from opening calcium channels in the sarcoplasmic reticulum) causes contraction. This signaling in contraction is a direct consequence of electrical activation of the voltage-gated channel. 

Signs and Symptoms Initial symptoms include fever, generalized weakness, back pain, dizziness, and extreme weight loss. There may also be occasional nausea and vomiting. Fever may be biphasic. In some cases the illness can become hemorrhagic or cause inflammation of the brain leading to headaches, coma, or seizures. Can also cause inflammation of the eyes with an intolerance to light. Up to 10% of affected patients may have some permanent vision loss. 

Initial clinical assessment involved some 1200 wet AMD patients. Although both control and product groups continued to experience vision loss, the rate of vision decline experienced by macugen-treated patients was significantly slower than in the case of control patients. The most frequent/potentially serious side effects noted during these trials were endophthalmitis, retinal detachment, eye inflammation/irritation and blurred vision, although rare cases of anaphylaxis have also been reported. Macugen is marketed by Eyetech Pharmaceuticals and Pfizer.
