All types of PHAs benefit from a multidisciplinary approach. This ensures appropriate technical expertise is available to evaluate each deviation discussed during the sessions. While it is not essential that all team members have had formal training in the PHA method selected, it is important that the team leader has experience. The participation techniques can be learned when a new participant is included as a part of a team in which some members have already gained PHA experience. 

Any designated, trained (8-hour HAZWOPER supervisor course as a minimum), and experienced individual responsible for the safety of an employee (such as team leaders or crew leaders) may perform the function of an experienced person to provide the supervised field experience required by HAZWOPER [1]. Although having the appropriate certificates of completion would satisfy regulatory requirements, you should also consider time of service and experience. A fresh college graduate with training certificates and minimal field experience may be less desirable to perform supervised field experience than the safety professional with years of substantial field experience. 

On the part of the contractor team, two root causes were identified, root cause 1 being insufficient training of the contractor team leader. He was uncertain of permit systems, specifically whether they should report in at the end of shift, and, if so, who should do it. The second root cause relates to the procedure used at the end of the shift for supervisors to sign back permits. Although according to procedure all workers should hand back permits to supervisors in person, in practice this did not occur. If no one is present, or they are busy, it had become common practice to either leave the permits on the supervisor s desk, or to sign them back in the morning. 

Recommendation sponsors, line managers, team leaders and HR/training might be able to suggest potential mentees. 

Using FTA requires a detailed understanding of how a process or system functions, detailed drawings and procedures, and knowledge of component failure modes and effects. The team leader should be well trained and experienced in constructing fault trees. 

Personnel with proper training, skills, and experience are critical to the successful outcome of an incident investigation. This chapter describes team composition as a function of incident type, complexity, and severity as well as suggested training topics. It also provides team leaders with a high-level overview of the basic team activities typically required in the course of conducting an investigation. 

External resources may he needed if the incident investigation work exceeds site capahilities. These resources could include corporate personnel or experts from outside the company. (The team leader may also he external if the incident is major since the leader s independence sets the tone for the investigation.) Company business unit leaders should confer with the team leader to determine whether external assistance is recommended. Factors to consider include significant offsite consequences such as environmental impact or product quality concerns. A team of trained specialists should formally investigate any process incident that could significantly affect the business. At the lower end of the scale, if a near miss or minor incident occurs that has no potential for significant consequences, local supervision or front-line personnel normally may perform the investigation without outside assistance.

Validation Team A well-defined validation team with a well-written description of responsibilities is required and assures the adequate realization of the validation tasks. A validation team should be composed by different responsibilities responsible-of-validation team, team leader, archive manager, test coordinator, quality assurance member, tester, and witness. The responsible-for-validation team elaborates and approves the VMP, protocols, and reports. The team leader should be responsible for the computer system validation and utilization. An archive manager is responsible for the management of all computer system validation documents. The test coordinator is responsible for the computer system test and coordinates the elaboration and operation of tests for evaluating the performance of the computer system. A quality assurance member is required to periodically inspect and train the personnel and review all the validation documents. The tester is responsible for the execution of the tests required to perform the validation protocol. The witness is responsible for observing and reviewing the operations of the tester. 

The review is a thorough and systematic process which has to be conducted in a proper fashion and accurately recorded. In order to perform a HAZOP review a specialized team leader is typically used to guide the review team during the process. The team leader usually has had specialized training and experience in the conduction of HAZOP reviews. 

The team leader should possess an engineering degree or equivalent. He should have a minimum of five years petroleum or related industry experience and be trained or experienced in the conduction of HAZOP or What-If techniques. A leader will typically have had three to five days of classroom training and have actually trained as a leader for one or two actual review sessions.
