All types of PHAs benefit from a multidisciplinary approach. This ensures appropriate technical expertise is available to evaluate each deviation discussed during the sessions. While it is not essential that all team members have had formal training in the PHA method selected, it is important that the team leader has experience. The participation techniques can be learned when a new participant is included as a part of a team in which some members have already gained PHA experience. [Pg.90]

Any designated, trained (8-hour HAZWOPER supervisor course as a minimum), and experienced individual responsible for the safety of an employee (such as team leaders or crew leaders) may perform the function of an experienced person to provide the supervised field experience required by HAZWOPER [1]. Although having the appropriate certificates of completion would satisfy regulatory requirements, you should also consider time of service and experience. Validation Team A well-defined validation team with a well-written description of responsibilities is required and assures the adequate realization of the validation tasks. A validation team should be composed by different responsibilities responsible-of-validation team, team leader, archive manager, test coordinator, quality assurance member, tester, and witness. The responsible-for-validation team elaborates and approves the VMP, protocols, and reports. The team leader should be responsible for the computer system validation and utilization. An archive manager is responsible for the management of all computer system validation documents. The test coordinator is responsible for the computer system test and coordinates the elaboration and operation of tests for evaluating the performance of the computer system. A quality assurance member is required to periodically inspect and train the personnel and review all the validation documents. The tester is responsible for the execution of the tests required to perform the validation protocol. The witness is responsible for observing and reviewing the operations of the tester. [Pg.835]

The review is a thorough and systematic process which has to be conducted in a proper fashion and accurately recorded. In order to perform a HAZOP review a specialized team leader is typically used to guide the review team during the process. The team leader usually has had specialized training and experience in the conduction of HAZOP reviews. [Pg.6]

The team leader should possess an engineering degree or equivalent. He should have a minimum of five years petroleum or related industry experience and be trained or experienced in the conduction of HAZOP or What-If techniques.
