To achieve these goals, we propose the following preliminary action plan. Establish a PSM Task Team. 

In addition, the U.S. Department of Health and Human Services (HHS) and other federal agencies formed the Quality Interagency Coordination Task Force in 2000 and issued an action plan for reducing medical errors. In 2001, former HHS Secretary Tommy G. Thompson announced a Patient Safety Task Force to coordinate a joint effort to improve data collection on patient safety. The lead agencies are the FDA, the Centers for Disease Control and Prevention, the Centers for Medicare and Medicaid Services, and the Agency for Healthcare Research and Quality. 

The next step in the business planning process is to define major milestones and an action plan of implementation and operation of the program. This action plan should detail the start and finish dates and list responsible individual(s) for each task necessary to accomplish the objectives of the business plan. The action plan should include periodic monitoring and assessment of the performance (i.e., clinical, financial, or other) of... 

Dr. Butler has created a service blueprint for the Med Check Program at Feel Great Pharmacy. She will also create a service blueprint for each of the Stay Fit Program components. These documents will help her to implement the new services in her pharmacy. Similarly, she has an action plan for one part ofher newprograms. Dr. Butler can add more details to the current action plan and later expand it to encompass both programs. This approach allows her to coordinate all the tasks that need to be completed for Feel Great Pharmacy to offer the new goods and services. 

The First Review Conference, however, went beyond this. It clearly recognized and highlighted the deficiencies that many States Parties had allowed to persist in their national implementation measures, and gave the Conference of the States Parties the task of developing and implementing an Action Plan to remedy the situation. The Conference subsequently adopted such an Action Plan. ... 

The mandatory requirement for all states to implement Security Council Resolntion 1540 is more than merely complementary to these efforts. It has added new depth and urgency to the task, and set short time frames for states to report on steps they have taken or intend to take to implement this resolution . The OPCW s Article VII Action Plan also seeks to have States Parties enact legislation, inclnding penal legislation, and adopt the administrative measures necessary to implement the CWC no later than the Tenth Session of the Conference of States Parties held in November 2005. These are ambitious targets, but the message to all states could hardly be clearer. 

It may seem premature to think about the monitoring and control of the European Action Plan at this stage. However, if such a plan is devised and implemented, continuous monitoring of developments within the sector on a European scale is essential. Up-to-date information must be available in order to identify potential problems which might reflect weaknesses in the plan. This monitoring, and reactions to it, could have institutional implications it is by no means certain whether the European institutions would be equipped to deal with this task, or whether reorganisation is necessary. This should be an issue for review. 

Another popular problem-oriented recording format used in many healthcare facilities today is the DAP (data, assessment, and plan) format. A brief format, the DAP encourages healthcare social workers and other professionals to identify only the most salient elements of a worker s client contact. Using the D (Data), social workers record objective client data and client statements related to the presenting problem and the focus of the therapeutic contact. The A (Assessment) is used to record the social worker s interventions, the client s reactions to the interventions, and the worker s assessment of the client s overall progress. Specific information on all tasks, actions, or plans related to the presenting problem and to be carried out by either the client or the worker is recorded under P (Plan). Information is also recorded here on issues related to the presenting problem that will be explored at the next session and the specific date and time of the next appointment. 

In dealing with accidents, incidents and emergencies several tasks have to be performed by the employer. First of aU he is obhged to prepare an action plan or an alarm plan which contains procedures to be followed if any such events occur. The Danish government in March 2000 appointed the Wilhjelm Committee. The task of the Committee was to prepare a report as a basis for a government action plan on biodiversity and nature conservation. 

It is also important that the accountability and reward systems for faculty and staff include attention to diversity and inclusion, such that employees are evaluated based in part on their effectiveness with respect to the university s diversity and inclusion action plan and their success in incorporating diversity awareness, knowledge, and skills into their everyday assignments and tasks. As an example, at Alliant, after adoption of the university diversity plan, the provost worked with the faculty senate to amend the criteria for faculty advancement and promotion to include the degree to which faculty incorporate multicultural and international topics and issues in their teaching and scholarship. In their annual self-study, faculty are asked to discuss whether and how their work has integrated multicultural and international issues, and their program directors, deans, and peer reviewers consider this in the evaluation decision. 

Implement Action Plan. An action plan is necessary to identify how the quality-improvement solution will be implemented. This includes defining the different tasks to be performed, the order in which they will be addressed, who will perform each task, and how this improvement will be monitored. Appropriate resources must again be identified and a timetable must be developed prior to implementation. Once the action plan is implemented, the indicators are monitored and evaluated to verify appropriate changes in the process. New indicators and thresholds may need to be developed to monitor the solution. 

Assignment—A task given to a resource to perform within a given operational period that is based on operational objectives defined in the incident action plan. 

Effective planiung and design of the workplace or job task can help to prevent hazards. Where it is not feasible to eliminate hazards, action planning can help to control unsafe conditions. 

Action plans can serve as a road map to move your safety program from where it is now to where it should be. An action plan defines what steps should be taken and the order in which tasks should be performed, and it identifies who is responsible for completing each task [5]. The plan must be specific and flexible enough to respond to change as the management systems and program grow and develop [5].
