Systems teams

For a major incident investigation using a comprehensive root cause analysis system, teams will be formed to acquire information relevant to determine the structure and analyze the causes in depth. In addition to evaluations of the immediate causes, imderlying causes are likely to be evaluated by investigations in areas such as safety and quality management. Both paper- and computer-based systems will be used to acquire and record information for subsequent detailed analyses. 

HACCP is best applied by a multidisciplinary team reviewing the design of a computer system. Team members are selected for their particular knowledge of the production process, the computer system, and the software.

ASAP was developed by CDM FPC s Expert Systems team, through a contract held by Camp Dresser McKee-Dallas with the Dallas Water Utilities. The domain expert, an employee of Camp Dresser McKee-Dallas, has over 20 years experience in the wastewater field, and works closely with Dallas Water Utilities. Development proceeded according to well-known expert system development guidelines, facilitated by the wastewater treatment experience of one of the knowledge engineers on the team. We estimate that her experience saved about 2 person-months in the 12 person-month development effort.

If the objective is to improve patient safety, we must rein in the tendency to blame and— the other side of that same coin— the tendency to look the other way or to cover up. Instead we must aim for a culture that is transparent and seeks to understand how best to improve critical systems, team functioning, the organization s safety climate, and safety leadership. A computer file of about 19,000 peak wavenumbers and intensities, along with search software, is distributed by the Infrared Data Committee of Japan (IRDC). Donated spectra, which are evaluated by the Coblentz Society in coUaboration with the Joint Committee on Atomic and Molecular Physical Data (JCAMP), are digitized and made avaUable (64). Almost 25,000 ir spectra are avaUable on the SDBS system developed by the NCLl as described. A project was initiated at the University of California, Riverside, in 1986 for the constmction of a database of digitized ftir spectra. The team involved also developed algorithms for spectra evaluation (75). Other sources of spectral Hbraries include Sprouse Scientific, Aston Scientific, and the American Society for Testing and Materials (ASTM). 

Within the larger system, research and especially development processes have become business processes, with teams from marketing, manufacturing, and finance participating from the start (8). In some companies, the R D function is so highly integrated with the business that formal R D organizations have vanished many corporate laboratories have been reduced or disbanded, with the researchers moved into separate business divisions (9,10). 

The detailed analysis, involving many respondents inside and outside the company, led to changes in the overall innovation process, ie, a company-wide priority system for innovation projects, measures of innovation for each functional and business area, training and supportive management systems for project managers, informal multifunctional teams in concept development and market development, a stmctured needs identification process, and appointment of a process steward to monitor the innovation process, measure how it functions, and coordinate innovation projects. 

The book does not focus on occupational safety and health issues, although improved process safety can benefit these areas. Detailed engineering designs are outside the scope of this work. This book intends to identify issues and concerns in batch reaction systems and provide potential solutions to address these concerns. This should be of value to process design engineers, operators, maintenance personnel, as well as members of process hazards analysis teams. While this book offers potential solutions to specific issues/concerns, ultimately the user needs to make the case for the solutions that provide a balance between risk... 

After the incident, an investigation team determined that the first operator had not added the initiator when required earlier in the process. When the relief operator added the initiator, the entire monomer mass was in the reactor and the reaction was too energetic for the cooling system to handle. Errors by both operators contributed to the runaway. Both operators were performing many tasks. The initiator should have been added much earlier in the process when much smaller quantities of monomer were present. There was also no procedure to require supervision review if residual monomers were detected. The lesson learned was that operators need thorough training and need to be made aware of significant hazardous scenarios that could develop. 

Employee Participation Preparation for startup is an excellent opportunity to document employee participation. If a toller is covered under the OSHA PSM regulation, they must record PHA participants, procedure writers and reviewers, PSSR team members and test run team members. All these roles reflect the true intent of this element of process safety. Eveiyone benefits from intimate involvement in the management system used to protect life and health. 

The tolling team has analyzed the hazards, addressed the risks and modifications using a management of change system, revised and written procedures, trained the workers, performed a PSSR, and completed any required test runs. The pre-startup phase is complete at this point. 

A system to address the team s findings, schedule and complete actions was not established.
