Adequate resources are fundamental to any project, but even more so to a well-functioning health and safety program. Many mishaps have been traced to improperly trained workers, lack of adequate tools and equipment, or requirements for personnel to work excessive hours or at unfamiliar jobs because of inadequate staffing or lack of adequate resources. The multidisciplinary team approach can help to identify required resources and can help balance, identify, and coordinate necessary assets. 

Employers must develop in-house capability to investigate incidents that occur in their facilities. A team should be assembled by the employer and trained in the techniques of investigation, including how to conduct interviews of witnesses, assemble needed documentation, and write reports. A multidisciplinary team is better able to gather the facts of the event and to analyze them and develop plausible scenarios as to what happened and why. Team members should be selected on the basis of their training, knowledge, and ability to contribute to a team effort to fully investigate the incident. 

HAZOP is a formal technique for eliciting insights about system behavior from a multidisciplinary team that collectively has thorough knowledge of the plant and the physical phenomena involved in the plant (Figure 3.3.4-1).

While a multidisciplinary team approach is ideally suited to PSM design and installation, it is not the only way. PSM goals can be achieved through the efforts of a single professional, or a small homogeneous task group (for example, facility managers or safety specialists) the process is essentially the same as described here, but on a smaller, more limited scale.

An H RA involves a review of the process design and its control, operation, and maintenance practices. The review is conducted by a multidisciplinary team with expertise in the design and operation of the process unit. The team uses a systematic screening process to determine how deviations from normal operation lead to process hazards. The academic base in experimental medicine, in both training and scienhfic programs, must be rebuilt. Translational research, that is, specific product-related research, alone is not sufficient. A government-academic-industry consortium or multiple consortia may be one model to get some of this research started, but clinical pharmacology needs to be integrated as a key part of the multidisciplinary teams in each of these endeavors. 

Because patients with very severe social problems tend to be excluded from prospective trials, additional problems may emerge now that many clinicians have accepted chemoradiation as the standard treatment for locoregionally advanced disease. There is no evidence that the use of concurrent chemotherapy permits significant reductions in total treatment dose there is also no evidence that chemoradiation overcomes the detrimental effects of treatment protraction. It is therefore critical that multidisciplinary teams work closely with patients and social services to educate patients and facilitate the timely and complete administration of this complex treatment.
