It should be remembered that humans are also prone to panic and confusion at the time of an emergency and so may also be unreliable in some instances. Where proper training and selection of personnel occurs situations of panic and confusion may be overcome. [Pg.177]

The selection of materials to be used in design dictates a basic understanding of the behavior of materials and the principles that govern such behavior. If proper design of suitable materials of construction is incorporated, the eqiiipment should deteriorate at a uniform and anticipated gradual rate, which will allow scheduled maintenance or replacement at regular inteivals. If localized forms of corrosion are characteristic of the combination of materials and environment, the materials engineer should still be able to predict the probable life of equipment, or devise an appropriate inspection schedule to preclude unexpected failures. The concepts of predictive, or at least preventive, maintenance are minimum requirements to proper materials selection. This approach to maintenance is certainly intended to minimize the possibility of unscheduled production shutdowns because of corrosion failures, with their attendant possible financial losses, hazard to personnel and equipment, and resultant environmental pollution. [Pg.2424]

Pilots and Igniters - Two gas-fired pilots with igniters are installed adjacent to the inlet distributor. There are two categories of equipment which determine the selection of equipment general-purpose and special-to-type equipment. It should not be necessary to specify all the general-purpose equipment needed to perform basic measurements, which would be expected to be known by appropriately trained personnel. You should not need to tell an inspector or tester which micrometer, vernier caliper, voltmeter, or oscilloscope to use. These are the tools of the trade and they should select the tool which is capable of measuring the particular parameters with the accuracy and precision required. However, you will need to tell them which device to use if the measurement requires unusual equipment or the environmental conditions prevailing require that only equipment be selected that will operate in such an environment. In such cases the particular devices to be used should be specified in the test or inspection procedures. In order to demonstrate that you selected the appropriate device at some later date, you should consider recording the actual device used in the record of results. The helidecks of offshore facilities are usually provided at the highest portion of the offshore installation for avoidance of obstructions during aircraft maneuvering and available space. As a result the roof of the accommodation is typically selected. This enhances one of the avenues of escape from the installation but also exposes the accommodation to several hazards.
