Safety operation instruction

Has each task that has been identified as critical in the area of quality and/or safety been documented with full operating instructions and are these instructions reviewed regularly ... 

In order to maintain a high level of quality and safety in the actual operations, whether chemicals are transferred, loaded/unloaded or transported, proper operating instructions/procedures are crucial and should be regularly reviewed. 

It should be expected that tasks for all operations are defined and those critical to quality and/or safety are identified. Critical tasks should have detailed operating instructions. 

The worker knew that valve A had to be closed. However, it was believed by the workforce that despite the operating instructions, closing B had a similar effect to closing A and in fact produced less disruption to downstream production. Possible cause violation as a result of mistaken information and an informal company culture to concentrate on production rather than safety goals (wrong intention). 

Information on the types of human interactions with hazardous systems that occur would be obtained from sources such as plant operating instructions, job safety analyses and similar sources. These interactions are referred to as critical tasks (CT). 

The design assessments and basis of safety must be recorded and available for consultation, and the process must be operated as required. The operating phase involves two sets of process documentation—the detailed technical documentation, and safe operating instructions (HSE 2000) for every operational phase, including startup, shutdown, maintenance and emergency operations. 

Product data information, material safety data sheets (MSDS), dosing and control equipment operating instructions, spares lists, minimum stock holding lists. 

There is a further non-trivial distinction between consumer products and the opportunities that arise from travel purchases. Most consumer products of some complexity come with an owner s manual. These kinds of documents provide operational instructions, safety hints, advice on replacement parts or persons to whom one can direct service inquiries. Perhaps the closest parallel in the world of tourist behaviour is the guidebook. Even here, however, there is a range of guide books for any one place. The holiday consumer is free to consult multiple owner manuals, to redesign and further refine his or her experience. Operating Procedures. Employers must develop and implement written operating instructions for safely conducting activities involved in each covered process consistent with the process safety information. The written procedures must address steps for each operating phase, operating limits, safety and health considerations, and safety systems and their functions. Included must be normal... 

All persons who work with producer gas generators are required to learn and conform to the necessary procedures for a safe and orderly operation. At each level of the hierarchical structure, inadequate control may result from missing constraints (unassigned responsibility for safety), inadequate safety control commands, commands that were not executed correctly at a lower level, or inadequately communicated or processed feedback about constraint enforcement. For example, an operations manager may provide unsafe work instructions or procedures to the operators, or the manager may provide instructions that enforce the safety constraints, but the operators may ignore them. The operations manager may not have the feedback channels established to determine that unsafe instructions were provided or that his or her safety-related instructions are not being followed. 

A number of requirements and pieces of equipment of lesser importance are embodied in the operating instructions and are designated as safety-related equipment. 

All appointed staff are therefore required to have an understanding of the Site Licence. The engineers appointed to Operating and Maintenance positions need to have an in-depth understanding and appreciation of Station Operating Instructions (SOIs), Plant Item Operating Instructions (PIOIs), Plant Maintenance Instructions (PIMIs) and other technical and safety related matters.
