Safety management systems Resolution

Assembly Resolutions in Force (as of 8 October 2004), ICAO Doc 9848, at 1-60. Implementation of Safety Management Systems (SMS) in States, Working paper submitted by the ICAO Secretariat, DGCA/06-WP/6, 9/01/06 at 1-2. [Pg.24]

The stories we tell should have a resolution. In the case of major events such as Piper Alpha or Deepwater Horizon, the resolution could be new ways of managing safety (Safety Cases) or the introduction of new regulations (SEMS). Even less dramatic stories should always provide guidance to better behaviors or improved management systems. [Pg.753]

Another example is tracking safety-related repair and maintenance activities. When a work request comes in, someone attaches an identifying bar code. The maintenance worker logs information into a management system about the item, problems, repair or maintenance needed, diagnostics, actions completed, and final resolution, such as approved for... [Pg.553]

Please describe at least three ways in which employees are meaningfully involved in the problem identification and resolution, or evaluation of the safety and health program (beyond hazard reporting). Are employees knowledgable about the site s safety and health management system ... [Pg.373]

Resolution of safety, security, health, environmental, and compliance-related concerns generated during all stages of review Tiered approval system in which the level of management approval required is dependent upon level of risk... [Pg.24]

Technical and engineering support is provided by TA-V analysts and designers. Maintenance and modifications of the equipment is performed by the facility operators, except for a major construction or modification or skilled craft work requiring a qualified plumber, electrician, or carpenter. If work on safety systems is performed by non-operators, the work is overseen by the operators and a checkout of the safety system is performed by the operators at the conclusion of the work to assure that the system(s) operates within specifications before operations are resumed. Facility Modification Requests (FMRs) are reviewed by an independent safety review committee, the Radiological and Criticality Safety Committee (RCSC), so that potential safety concerns are identified and referred to the proper level of line management or the DOE for resolution and approval. [Pg.368]

A second important similarity between the Sustainable Safety and Vision Zero approaches is the shared responsibility of road user and system manager/designer/ operator. Both visions embrace the concept of shared responsibility instead of blaming the victim , as used to be the case in other approaches in the past. Shared responsibility also implies good cooperation between different tiers of the government. This is a component in both visions. In the Netherlands this resnlted in a covenant between the three layers of government with a considerable financial impulse to execute the associated resolutions. [Pg.420]

The Conduct of Operations Manual requires shift operations personnel to be attentive to system conditions and parameter trends by monitoring the information displays associated with each person s duties. It states that operations personnel should remain diligent and actively pursue resolution of safety concerns. Shift operations personnel are required to constantly remain alert and maintain awareness of plant status, anticipating conditions that could adversely affect plant reliability. The manual states that operators shall be alert and attentive to alarms and, if appropriate, shall take timely action to address and correct the cause of alarms. Additionally, it states that operators shall monitor the panel displays frequently to determine trends and to detect the development of problems. Operators are required to take prompt actions to determine the cause of the abnormalities, advise shift management of the abnormalities, and take corrective action. [Pg.600]

Describe details of how resolution and action relative to system safety will be executed at the program management level possessing resolution authority. [Pg.52]
