Rheumatoid illness

Primaquine should be administered cautiously to actually ill patients with any serious systemic disease characterized by a tendency to granulocytopenia such as rheumatoid arthritis or lupus erythematosus. The drug should be used with care in patients with glucose-6-phosphate dehydrogenase deficiency. Primaquine should be withdrawn if signs of haemolysis or methaemoglobinaemia occur and the blood... 

After I had partially recovered I got board certified in environmental medicine. Now I m certified in both family practice and environmental medicine. I think most people are affected by chemicals. Those of us with MCS are just on one end of the spectrum. I see patients with MS, rheumatoid arthritis, fibromyalgia and other conditions, and there s almost always a chemical link someplace in their complaints. I always keep an eye out for that aspect contributing to these illnesses. 

This point of view overlooks the fact that every well and normal individual is potentially an ill individual, and the roots of disease may be present in his make-up years before there is any overt disease. A dozen young men used as normal controls may each have metabolic peculiarities that point toward a different metabolic derangement gout, multiple sclerosis, diabetes, anemia, atherosclerosis, hypertension, nephrosis, hypothyroidism, rheumatoid arthritis, rheumatic heart disease, liver cirrhosis, and myasthenia gravis, for example, and yet at the time of their use as controls these young men may show no symptoms of the disease which is to appear later in life. It seems far from safe to assume that because an individual on clinical examination seems well, all of his blood values, for example, are normal and meaningless so far as disease susceptibilities are concerned. 

The degree of study subject tolerability to a drug should be assessed in conjimction with the laboratory safety and efficacy data, so that an overall risk to benefit assessment can be made. Poorly tolerated drugs, however efficacious for use in self-limiting non-serious diseases, are unlikely to become successful medicines. On the other hand, study subjects with serious illnesses, such as active rheumatoid arthritis, are frequently quite prepared to put up with poorly tolerated drugs (e.g. intramuscular gold injections or intra-articular steroid injections) if efficacy is good and the alternatives are no more attractive. 

Corticosteroids do not heal illnesses, but they are widely used in various conditions when it is necessary to utilize their anti-inflammatory, immunosuppressant, and mineralo-corticoid properties. In addition, they are used in replacement therapy for patients who have adrenal insufficiency. Corticosteroids can be used in vital situations for asthma, severe allergic reactions, and transplant rejections. They are effective in noninfectious granulomatous diseases such as sarcoidosis, collagen vascular disease, rheumatoid arthritis, and leukemia. Steroids are used as lotions, ointments, etc. in treating a number of dermatological and ophthalmologic diseases. 

Autoimmune disease—One of a number of illnesses caused by the immune system targeting the patient s own tissues for destruction, such as rheumatoid arthritis or multiple sclerosis. 

Levamisole (see Chapter 26, Section Ill.e and Chapter 28, Section III) is an imidazothiazole derivative and the L isomer of D,L-tetramisole. It has activity against Ascaris and Trichostrongylus but is mainly used for its immunomodulating effects in Rheumatoid Arthritis and as adjunct therapy in some anti-cancer regimens.

DFOA has been examined for ability to inhibit chronic inflammation. In a preliminary study with rheumatoid patients, intramuscular DFOA (1 g) produced an abrupt rise in haemoglobin, a response which was taken to indicate suppression of inflammation77. Under this regime no ill effects were observed but this was not the case where rheumatoid patients received 3 g d 1 for 5 days each week over 1 to 3 weeks78. Out of seven patients ocular abnormalities were induced in four of them but subsided on termination of DFOA treatment. Further complications of DFOA therapy were observed with two patients who lapsed into unconsciousness for up to 72 hours when they were placed upon a course of treatment with phenothiazine, a treatment initiated to suppress DFOA-induced nausea. 

Systemic lupus erythematosus and rheumatoid arthritis The possible role of interferon alfa in the development of rheumatoid arthritis or systemic lupus erythematosus has been described in isolated cases (363,364), and confirmed cases of systemic lupus erythematosus have very occasionally been reported (SED-13, 1096) (SEDA-20, 330). In most of these cases, the predominance of young patients and female sex, the presence of renal or skin involvement, the findings of positive antibodies to double-stranded DNA, and the rapid onset after the start of treatment, as well as persistence of symptoms after interferon alfa withdrawal, are more in keeping with unmasking by interferon alfa of idiopathic lupus rather than with a new drug-induced illness. The reactivation or appearance of inflammatory rheumatological disorders consistent with rheumatoid arthritis or lupus-like polyarthritis were... 

Metals A wide range of metals induce nephrotoxicity in humans and/or in animal models (Table 5). Some of these metals (e.g., iron, cobalt, copper) are essential elements required for normal body function, while others can be useful in treating diseases. For example, gold salts are useful in treating rheumatoid arthritis lithium salts are indicated for the treatment of manic-depressive illness and aluminum and bismuth salts are available to treat indigestion and stomach aches. However, exposure to these and other metals can occur from environmental sources and in excessive concentrations, can lead to nephropathy. 

This invention is based on the discovery that Al-THC-7-oic-acid is a potent analgesic and anti-inflammatory agent, and that when administered directly into the stomach is non-ulcerogenic. As a result, this non-psychoactive metabolite of THC can be used as a therapeutic agent for such purposes as the treatment of chronic pain and tissue inflammation often associated with illnesses such as rheumatoid arthritis. 

The present invention is generally directed to non-psychoactive derivatives of A6-THC-7-oic acid, which have been shown to be potent analgesic and anti-inflammatory agents and to possess leukocyte antiadhesion activities. The invention is further related to the use of these derivatives as therapeutic agents in the treatment of pain and tissue inflammation, especially that associated with long-term illnesses such as rheumatoid arthritis.
