Hydroxyurea is an older agent still used occasionally today for patients with psoriasis however, there have been recent precautions about its use in the elderly and cutaneous vasculitic toxicities in patients with myeloproliferative disorders.29 Toxicity associated with tacrolimus has limited its use in psoriasis. Azathioprine has a slow onset and significant toxicity.29 Oral corticosteroids are reserved for severe or life-threatening conditions such as severe psoriatic arthritis or exfoliative psoriasis prolonged oral steroid use should be avoided.10...

Erythrodermic psoriasis results in a widespread erythema that affects most of the body. It is often associated with pustular psoriasis. Oedema, particularly around the ankles, is common, as is excessive exfoliation of the skin and severe itching and/or pain.

A dose-escalation study in 35 patients with psoriasis confirmed that constitutional symptoms in response to denileukin diftitox were dose-related and less frequent at lower doses (below 5 micrograms/kg/day) (17). There was only one case of mild vascular leak syndrome. Skin reactions compatible with delayed hypersensitivity reactions were noted in three patients, including one case of exfoliative dermatitis. 

Other skin reactions that have been reported include iododerma (200), erythema nodosum, psoriasis (201), and exfoliative dermatitis (202). 

Severely ulcerated psoriatic plaques and acute extensive exfoliative dermatitis occurred in a 37-year-old man who had taken methotrexate for 5 years for psoriasis (74).

People with other acute, chronic, or exfoliative skin conditions (such as bums, impetigo, varicella zoster, herpes, severe acne, severe diaper dermatitis with extensive areas of denuded skin, psoriasis). 

Psoriatic lesions are relatively asymptomatic however, pruritus is a complaint in about 25% of patients. Severe, widespread psoriasis may involve symptomatology similar to that of exfoliative dermatitis, which may include fever and chills. Psoriatic arthritis is a distinct clinical entity in which both psoriatic lesions and inflammatory arthritis-like symptoms occur. Classically, distal interphalangeal joints and adjacent nails are involved, but knees, elbows, wrists and ankles also may be involved.

Either history or evidence of eczema. Other exfoliative or extensive skin lesions (eg, atopic dermatitis, psoriasis, burns) may also place a person at increased risk for vaccination. 

Skin A 67-year-old woman with rheumatoid arthritis and psoriatic arthritis developed a full-body exfoliating pustular rash with fever, confirmed on biopsy as generalized pustular psoriasis, it responded dramatically to methylprednisolone [56 ]. 

Psoriasis is a chronic proliferative inflammatory skin condition. The severity of the condition ranges from mild focal involvement to generalised exfoliative dermatitis. The clinical course is also unpredictable. 

This is a disorder in which exfoliative erythroderma is present at birth or occurs during early childhood, and continues into adulthood. Gross and histologic features most closely resemble psoriasis (Figure 13.8). A family history of psoriasis may be present. 

After 3 weeks, 15 of the 26 patients were improved, thou none of them cleared completely. A common side effect (after 13-cis- retinoic acid only) was exfoliative cheilitis. In a few cases headaches, diarrhoea, vomiting, and increases in sedimentation rates, transaminases and serum lipids were noted. Schimpf (86 ) treated a group of 33 psoriasis patients with the vitamin A acid derivative Ro 10—9359 (ethyl-all-trans-9(4-methoxy-2,3,6-trimethylphenyl)-3,7-dimeth-yl-2,4,6,8-non-atetraenoate). Furthermore 13 patients with keratoses were treated systemically with Ro 10—9359 the period of treatment varied from 28 days to 9 months.
