Proactive Treatment

This is required to mitigate as much as possible the effects of the toxic agent in producing acute lung injury, toxic pulmonary oedema and possible ARDS (see Chap. 9). 

Early type 2 ventilation failure requires a life-support strategy to clear the airway and start ventilation. The use of a volume preset ventilator is important to ensure the delivery of an adequate tidal volume since there may be decreased compliance (V/P) and increased airway resistance with the development of type 2 respiratory failure. 

If there is a developing respiratory failure of the type 1 variety due to acute Itmg injury and the gradual infiltration of the alveoli, pressure support ventilation may be required. This is normally provided in an intensive care unit as part of a protective ventilation strategy used in the management of ARDS (see Chap. 9).

If they are to be effective, steroids should be given parenterally as soon as possible after exposure. Once pulmonary oedema has set in, steroids have been shown to be less effective.

Today, the physician of mainstream medicine still does not even remotely have the time to take all lifestyle factors into account (and certainly not provide any broader guidance) without the resource that IT could provide. The best that can be done is referral to a specialist, which may never happen. Neglect of a holistic view is especially noticeable when a patient s illness demands urgent attention, and all effort must be focused on the episodic and reactive (instead of proactive) treatment of the patient. Most often a so-called disease is technically a symptom, of an underlying cause that is not addressed. Symptoms are cured, but by waiting until the patient reaches a state where treatment is essential, the core problem is harder to treat. If truth be told, the inevitable and fundamental nature of any episodic and illness-oriented approach allows a patient to deteriorate to a condition of stress before action is taken. It is even defined as the moment at which the patient is so stressed that he or she takes the decision to be a patient, and often the point of illness is all that the physician ever gets of a glimpse of the patient. 

The objective of risk management and a safety culture should be a proactive treatment of clinical risk and therefore minimisation of adverse incidents arising from HIT. Nevertheless this should not detract from the notion that one can learn a great deal from analysing incidents or near misses. A patient safety incident is an event or circumstance which could have resulted, or did result, in unnecessary harm to a patient [22]. The ongoing study of incident reports can help identify common types of problems [23] and incident reporting has been cited as one of the major steps in improving the safety of HIT [24]. 

Once VLS has been stabilised with topical corticosteroids, twice-weekly proactive application of mometasone furate 0.1% ointment over 56 weeks was found to be an effective and safe therapy option in maintaining VLS remission and in preventing the occurrence of relapse. These results appear to indicate that proactive treatment with a topical corticosteroid twice weekly may be effective, safe and reliable therapy for the long-term prevention of relapse of VLS in patients previously treated with topical corticosteroid, and for maintaining VLS remission. 

It is important to point out that the proposed control strategy allows to handle uncertainty and incidences by combining reactive and preventive approaches. A proactive treatment of uncertainty is included by means of stochastic programming. The review and update process that are required to deal with incidences and changes in random factors are performed by introducing the SC stochastic holistic model into the MPC framework.

To achieve this objective at the operational level, consistently good water treatment practice is required on site. However, achieving this objective cannot and should not be the sole prerogative or responsibility of any one person. Rather, if success is to be attained and maintained, it requires a proactive approach with joint participation, support, and communication by the water treatment service company and the owners and/or operators of the boiler plant. 

In addition to supplying the various water-steamside conditioning equipment, chemical treatments, and periodic cleaning or other specialist services, service companies are expected to provide advice, practical expertise, and general support for their program in a proactive manner, to prevent waterside and operational problems from occurring. 

Thus, the entire thrust of a water treatment program requires a proactive approach to potential problem identification and resolution. The scope of the program should therefore be wide enough to include not only all relevant conditioning equipment and chemical treatments but also a variety of technical and operational support services. 

The function of boiler water treatment, therefore, is to control the waterside chemistry of boiler plant systems within certain agreed and relevant parameters and specifications. As these adverse processes are by no means limited to the boiler itself, in practice, boiler water treatment also includes pre-boiler and post-boiler functions and further requires that all the various types of water utilized are controlled through a comprehensive treatment and proactive management program. 

Optimally, both sex partners should be treated simultaneously for a STI however, this is difficult to accomplish. Clinics and health departments often proactively attempt dual treatment by providing a prescription for the partner to the index patient, a practice commonly known as expedited partner treatment . 

Synthesis of Subsection 3.2.4 - Only a minor fraction of sprayed pesticides hits the intended target, while most of them pollute the environment however, pesticides cannot be abandoned. IPM helps to enormously reduce their use and is presently progressing towards the proactive approach . The only division in CDER with an established policy regarding pre-IND consultation meetings is the Division of Anti-Viral Drug Products (DAVDP). Furthermore, the recognition of variations across the different scales of spatial and temporal dimensions would enable the identification of shifting therapeutic targets to address both of the individual and the time variances in personalized medicine (see Fig. 1). Accurate and robust biomarkers can also be useful for the stratification of diseases and classification of patient subgroups for more effective prevention and therapy. The prediction of drug responses would in turn help avoid adverse events for better clinical outcomes. In addition, the construction of dynamic disease predictive networks derived from the analyses of omics data would allow for the transition from reactive treatments to holistic and proactive care. With the transformation from disease-centered to human-based care, the systems and dynamical models would provide patient-centric information to enhance the participation of individuals, the goal of participatory medicine. 

Correspondingly, this work had a proactive component to it. Much of the work within the DoD core research program addressed the question, Is the current US military medical treatment doctrine, as well as physical protective measures (protective masks, clothing, and support systems), adequate to protect soldiers in future deployments from effects of exposure to low-levels of CW agents (GAO/NSIAD, 1998). 

There is a strong dose-response relationship between the intensity of tobacco dependence counseling and its effectiveness. Treatments involving person-to-person contact (via individual, group, or proactive telephone counseling) are consistently effective, and their effectiveness increases with treatment intensity (e.g., minutes of contact).
