The public inquiry into the Herald of Free Enterprise disaster was an important milestone in ship safety in the United Kingdom. It resulted in actions such as changes to marine safety-related rules and regulations, the introduction of the International Safety Management (ISM) Code for the safe operation of ships and for pollution prevention, and the development of a formal safety assessment process in the shipping industrial sector [17]. 

Specific concentration limit refers to a critical gas concentration limit that should not be exceeded. This limit can be a Threshold Limit Value (TLV), Time Weighted Average (TWA), Permissible Exposure Limit (PEL), Short Term Exposure Limit (STEL), Lower Explosive Limit (LEL) or Immediate Danger to Life and Health (IDLH) limit. These limits can be defined in terms of percent volume (%), parts per million (PPM), or even parts per billion (PPB) Table 1 lists hazards and exposure limits for several common hazardous gases foimd in the electronics industry. Critical gas concentration limits may be mandated by environmental, health and safety statutes, regional or local codes, local code enforcement officials, internal risk management personnel, or an established organizational policy. 

Maintenance costs form a significant part of the overall operating costs in maritime operations. Maintenance also affects reliability and can thus have environmental and safety consequences. The International Management Code for the Safe Operation of Ships and for Pollution Prevention (ISM Code) addresses the management aspects. The importance of maintenance is demonstrated by the fact that it is the only shipboard activity to have one whole element assigned to it (i.e. ISM Code element 10) (IMO (1997)). 

As previously stated, the American Chemistry Council also recognizes this concept as an integral part of the code of Responsible Care , Process Safety Code of Management Practices, Companies should openly share nonproprietary results from internal process safety incident investigations and related research through the auspices of trade and professional associations, and other networking opportunities. Other organizations should strive to benefit from the lessons learned by others and keep abreast of latest developments and safety alerts. 

In the 1980s, Responsible Care s six Codes of Management Practices served as a framework for the chemical companies to use internal processes to improve performance. Overall, environmental performance did improve because of a commitment to the codes and expanding regulatory requirements (Yosie, 2003). Members of the CCPA steadily improved their environmental and workplace health and safety records throughout the 1990s. Their records indicate a steady decline in workplace injuries and a marked reduction in the frequency and severity of transportation incidents (CCPA, 1999).
