Instances of chemical accidents, like to those of Section 7.1, relative to the hazards listed in Step 3 are listed. Information for these must come from various souices since there is not a single database for chemical incidents. If the plant has operating experience, this is valuable information, although the data may be sparse. 

As noted earlier, the obvious limitations of a system based on information reported on MSDSs and hazard lists are that MSDS information can be incomplete and inaccurate and screening hazard lists does not reflect a complete hazard assessment. Assessment. An analysis of the hazards present in this laboratory show the most significant hazard to be the release of vapor CSM from engineering controls and into the workplace. Preliminary Hazard List Description. The incorporation of this information into a PHL entry is shown as Table I. This entry describes the nature of the hazardous event (column 1), why or how the hazard may result in a mishap (column 2), the effects on operating personnel, equipment, and the facility (column 3), the risk assessment code assigned to the uncontrolled hazard (column 4) and any comments the originator may have (column 5).

A waste stream may be defined as hazardous under RCRA if it meets certain criteria for ignitability, corrosivity, reactivity, or toxicity, or if the waste stream is specifically identified by EPA as a hazardous waste. An individual waste stream is subject to being classified as hazardous (listed) if it contains any one of approximately 375 chemicals identified by EPA as hazardous constituents. These designated chemicals are frequently referred to as Appendix Vlll compounds because of where they are listed in the published regulation. 

These and other factors must b6 examined before a method or apparatus is condemned for lack of agreement with some other method or instrument. It is often simpler and more definitive to establish that an apparatus is properly working and calibrated than it is to use reference standards which are subject to the hazards listed above. Furthermore, the question must always arise regarding the merits of the technique and apparatus used to establish the value of the standard in the first instance. Experimental results obtained with a carefully calibrated apparatus are preferred to results which may agree with a presumed standard whose properties may change or which may have been incorrectly measured in another laboratory.

A PHA was prepared as part of the Parsons/Honeywell EDP, in accordance with MIL-STD-882C. The PHA is based on a preliminary hazards list that describes unmitigated hazards the list will be updated as the design process progresses (DOD, 1993). Numerous PHA work sheets were generated, and recommendations were made to reduce risk. The PHA results were reviewed by a multidisciplinary group from the Parsons/Honeywell team, which will track the means of mitigating hazards.

The first order of business on the plan should be the immediate rectification and elimination of the hazards listed in the baseline audit report. A typical system safety task for hazard identification would be the preparation of a preliminary hazard list (PHL). Hazard identification or discovery is accomplished by reviewing lessons learned, accident reports, and other historical data. A PHL may be prepared through an informal conference, the use of checklists, and occasionally other techniques such as energy trace and barrier analysis (ETBA). 

Purpose. The purpose of Task 201 is to compile a preliminary hazard list (PHL) very early in the system acquisition life cycle to enable the MA to choose any hazardous areas on which to put management emphasis.
